Job Title:Registered Nurse (Home Health) - Taos, NM_Full Time Perm

Shifts: Monday - Friday, 8:00 AM to 5:00 PM, with flexibility based on patient needs.

• Be available for on-call duties on weekends, with on-call pay of $75 per day

• patient ratio of 4-6 per day

Description:

We are hiring experienced Registered Nurses for Home Health and Hospice roles. Candidates should have 1-2 years of relevant experience, proficient in Kinsser/WellSky.

Responsibilities:

• Conduct comprehensive assessments of patients in their homes, ensuring individualized care plans are developed and implemented.

• Administer skilled nursing care to patients requiring various medical treatments and interventions.

• Collaborate with interdisciplinary teams to coordinate and manage patient care effectively.

• Utilize Kinsser/WellSky for documentation and communication to ensure accurate and timely record-keeping.

• Provide support to patients ranging from middle age to geriatric, adapting care plans to meet their unique needs.

• Maintain a ensuring personalized attention and quality care delivery.

• Respond promptly to changes in patient conditions and modify care plans accordingly.

• Collaborate with other healthcare professionals to ensure a holistic approach to patient care
  Qualifications:

• RN license and CPR certification

• 1-2 years of Home Health and Hospice experience

• Proficiency in Kinsser/WellSky preferred

• Strong communication and interpersonal skills
