Join CentraCare’s “Team Teal” as a Home Care Registered Nurse (RN), where you’ll serve people in the community, support Field Nursing staff in their roles, and Home Health patients from an office setting. You will be able to be involved in the intake of new patients, taking calls from patients and family members, and providing support to the team throughout the day.

Superb Schedule:
- Part-time, 16 hours every two weeks.
- Mostly rotating weekend shifts working 8 a.m.-4:30 p.m.
- Every 4th weekend.
- Some weekday shifts.
- No Holidays.
- No On- Call.
- Evolving role that may need to be adapted in future.
Duties Include:
- Collaboration with other team members.
- Use of technology, telemonitoring, computer, EPIC, and Phone system.
- Scheduling of Home Health Visits and Admissions.
- Providing education to patients and family.
- Triaging patient and family calls.
- Coordination with Providers and other members of team.
Pay:
You’ll make more but our minimum pay is: $34.04/hr; BSN $34.89/hr; MSN $35.76/hr
Qualifications:
- Graduate from an accredited school of nursing required.
- Current Minnesota Registered Nurse license required.
- Current American Heart Association (AHA) BLS Healthcare Provider card required.
- Minimum of one year of RN experience in long-term care or an acute care setting preferred.
- Valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage required.
Sign-on bonus requirements: To be eligible for the sign-on bonus you must be an external candidate who is hired into an eligible full time or part time benefited position. Previous employees qualify if they have not worked for CentraCare/Carris for at least 9 months at the date of hire.
