Licensed Practical Nurse (LPN) for Office/Intake Staff

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join our home care team in an Office/Intake role, with occasional fieldwork as needed. The ideal candidate will be responsible for managing intake processes, making sure insurance paperwork is in order, coordinating care, and ensuring the highest level of service is provided to our clients.

Key Responsibilities

Intake Coordination:

• Conduct initial client assessments and gather necessary health information and history.
• Coordinate with healthcare providers to process orders.
• Manage and process new client referrals.
• Ensure all client documentation received is accurate and complete.

Office Administration:

• Assist with daily administrative tasks, including scheduling, answering calls, and managing client records.
• Collaborate with the Start of Care team to facilitate seamless care coordination.
• Provide information and support to clients and their families.

Client Care (as needed):

• Provide skilled visits in the field when required.
• Medication Management and monitor client health status.
• Educate clients and their families on health care and treatment plans.

Compliance and Quality Assurance:

• Ensure compliance with all health care regulations and standards.
• Participate in quality assurance and improvement initiatives.

Qualifications:

• Current LPN license in Minnesota
• Minimum 1 years of nursing experience, preferably in a home care setting.
• Strong organizational and communication skills.
• Ability to work independently and as part of a team.
• Proficiency in electronic medical records and office software.
