LVNs are primarily responsible for providing nursing care to patients in their homes, ensuring that they receive the highest quality of care and support.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Able to do field visits as needed by the patient
- Case Management by providing the adequate patient and family/caregiver education about hospice philosophy in order to comply with the plan of care needed for patient's diagnosis
- Provide adequate education on appropriate usage of medications, DME, and supplies
- Ensures all patient concerns are addressed and coordinated to bring in best comfort care possible
- Communicate with physician, caregivers, family, hospice team members with regards to any change's in patient's condition
- Be familiar with Government guidelines
- Completes all reports in a timely manner
- Maintains confidentiality in accordance with HIPAA compliance
- Participates in Interdisciplinary Team meeting and discuss appropriate nursing intervention or changes in plan of care as needed
- Participates in On-Call rotation as needed and requested
- Performs job in compliance with Agency policies, procedures and applicable laws
Requirements for Position:
- Graduate of Bachelor's Degree in Nursing
- Valid LVN/RN License
- Valid California Driver's License
- Reliable Transportation
- Proof of Auto Insurance
Bilingual: Spanish, Armenian speaking is a plus
