Job Description
Our Clinical Assistant’s primary function is to assist the Clinical Manager, schedule patient services, and coordinate with staff.
Job Responsibilities
- Schedules and appropriately documents patients schedules in a timely manner.
- Participates in coordinating care with management and patient interdisciplinary team.
- Provides effective communication to patients, staff members, other healthcare professionals, and referral sources.
- Responds to Agency and patient needs in a professional and creative manner.
- Demonstrates commitment, professional growth, and competency.
- Promotes the Agency philosophy and administrative policies to ensure quality of care.
- Reaches out to and follows up with doctor offices in regard to patient care.
- Manages the flow of patient care.
- Communicates with the pre-authorization team.
- Discharge patients from care under the direction of the Clinical Manager.
- Send lab referrals to the approved destination.

Job Requirements
- Prefer an LVN License
- Previous experience in a Home Health setting (preferred)
- Previous scheduling experience (preferred)
- Customer Service experience in a high volume setting (required)
- Reliable transportation (required)
