Job Description Summary
The Home Care and/or Hospice Facilitator is responsible for maintaining positive relationships with customers and referral sources, meeting with patients and families and facilitating a patient's smooth transition to Home Care or hospice.
Essential Job Functions/Responsibilities
- Establishes and maintains positive working relationships with current and potential referral sources
- Monitors community, customer, payor and patient perceptions of the Agency as a high quality provider of services
- Maintains a working knowledge of Home Care and/or hospice
- Provides input by identifying opportunities for additional or improved services to address customer needs
- Maintains a working knowledge of community resources and assists customers in accessing community resources.
- Consults with physicians, nurses, social workers, discharge planners and other disciplines to establish a coordinated plan of care
- Meets with patient, family, caregivers and discusses the patient situation, current needs, and any psychosocial factors that are relevant to the plan
- Completes referral information that includes intake data, essential background information and inpatient care received to facilitate the transition of the patient to Home Care or hospice
- Maintains excellent communication with customers and referral sources regarding the discharge process. Keeps agency abreast of any changes or delays in the discharge to home or transition to hospice
- Keeps referral source informed of any re-hospitalization following the patients discharge to home or transition to hospice
- Attends meetings and participates on committees as required
- Actively participates in the planning, organizing and staffing of events, activities, wellness clinics, health talks and screenings
- May participate in marketing meetings and strategic market planning
- Monitors promotional material and collateral inventory and assembles needed marketing materials
- Back up to intake department when needed
- Performs other duties as assigned

Position Qualifications
- Knowledge of Home Care and/or hospice preferred
- Ability to deal tactfully with customers and the community
- Conduct self as a professional representative of the Agency
- Demonstrates good communications, negotiations, and public relations skills
- Demonstrates autonomy and must be a self-starter, attentive to detail and possess excellent organizational skills, assertiveness, flexibility and cooperation in performing job responsibilities
- Ability to stay motivated
- Must be able to work with little supervision
- Must be flexible with schedule demands and possess a positive attitude and approach to change
- May be required to work an occasional weekend and or evening
