SUMMARY: A Home-Based Family Coach conducts weekly home visits to enrolled families focusing on implementing Community Progress Council's Coaching Model with the family and providing educational and developmental support to enrolled children. The Home-Based Family Coach will use the Parents- As- Teacher's curriculum to work with pregnant individuals and parents to develop individual goals for children, provide on-going assessment on progress and facilitate transition into the next caregiver experience outside of the home. Family Coaches will support families in developing goals towards self-sufficiency and participate actively in supportive coaching to help them achieve these goals.
ESSENTIAL FUNCTIONS
- Be committed to the vision and mission of and Community Progress Council and Head Start of York County.
- Provide individualized, culturally sensitive support to child and family using a relationship-based and client-centered approach and coordinate services internally and externally.
- Have a full understanding of CPC programming requirements and maintain a full understanding of eligibility.
- Provide coaching to parents and children in the home using approved curriculum to support families in understanding developmental principles and practices.
- Prepare and maintain required lesson plans, observations of children, developmental screenings, health screenings, attendance, and other documents and record.
- Complete holistic assessments to help address family strengths, needs and/or concerns.
- Work with family to identify goals and provide a coaching experience that is focused and motivating to clients at a location that is convenient for them.
- Develop and maintain effective working relationships with diverse individuals, including children, families, co-workers, staff in related agencies and the general public.
- Demonstrate knowledge of pregnant persons, Human Development, Early Childhood theory, child development, developmentally appropriate practices, and behavior management.
- Assist in scheduling, planning and implementing two socialization groups per month by involving parents and family members in planning; develop and maintain a clean, safe and positive classroom environment that promotes children and family members to interact, learn, practice appropriate and acceptable behaviors; encourages families to network; models responsive caregiving.
- Provide transportation to clients in CPC vehicles as applicable.
- Complete timely and accurate data entry; maintain accurate records, collaborate with internal programs and ensure client confidentiality.
- Collaborate as part of comprehensive service team to families including health, education, and inclusion.
- Facilitate parent engagement activities for caseload as needed, including but not limited to planning, sending letters/reminders, organizing speakers, executing meetings/groups, etc.
- Recruit eligible pregnant individuals, children and their families, support enrollment process.
- Vary work schedule to accommodate parents' schedules and to attend program functions.
- All other duties as assigned.
EDUCATION AND OTHER REQUIREMENTS
- Bachelor's degree (B. A.) in Early Childhood, Child, and Family Studies or Human Development; Home Visitor Child Development Associate credential (CDA); RN; LPN; Certified Home Health Aide.
- One-year related work experience with expecting mothers, infants, and toddlers preferred.
- Or an equivalent combination of education and experience.
