A High Intensity Youth Treatment (HIYT) Care Coordinator is needed to help us in our endeavor to build the healthiest community in the nation … one person at a time.

  • Location: Larimer County, CO
  • Status: Part-time, 20/hrs. week 
    • *can be paired with the HIYT Transportation Driver role for full-time employment

What You’ll Do:

 

  • As a Care Coordinator - Youth Liaison, this role is responsible for providing care coordination services on the High Intensity Youth Treatment Team to engage individuals requiring behavioral health and/or substance use services. This position will work closely with the School Based Mental Health Specialist and collaborate with client and required health systems to provide support, link to resources, and fill treatment gaps as needed. Clients may be transitioning from emergency departments, health services, primary care settings, emergency response services, acute care systems, crisis response teams, and behavioral healthcare teams.This role will travel to area schools and provide support for problem solving in addition to providing training, resources, and support to educators.  They will also field questions about adolescents and Substance Use Disorder, and work with parents, guardians, and caregivers to provide support and guidance as they navigate school services. 

     

    Other duties include:

    • Providing care coordination services on behalf of and in collaboration with clients.
    • Engaging with various stakeholders and community partners to coordinate care including but not limited to emergency departments, health services, primary care settings, acute care systems, crisis response teams, and behavioral health teams. 
    • Outreach clients on a referral and/or cold outreach basis. 
    • Survey clients utilizing SAMHSA survey tool to monitor program participation.  
    • Engage in Certified Addiction Specialist coursework.  
    • Collaborate with SummitStone staff and the Colorado Opioid Synergy Larimer and Weld (CO-SLAW) network. 
    • Support clients navigating health systems and increase continuity of care.  

     

 

Required:

  • High School diploma, GED, or equivalent vocational or short-term courses in behavioral health; 1+ years of relevant experience; Valid Colorado driver's license; CAS certification required within 24 months of hire (training provided)
  • Skills: computer proficiency, organizational, attention to detail, communication; Knowledge of diversity and inclusion with the relevant ability to engage, communicate, interact, and work effectively and cooperatively with people of different backgrounds, identities, and culturally diverse communities.

 

Preferred:

  • Bilingual/Bi-cultural; Bachelor's degree in psychology, social work, or related field from an accredited college or university; Trauma Informed Care experience; CAT or CAS.

Some positions include driving a company vehicle and require a valid Colorado driver’s license.

The typical hiring range for this position is $21.50 - $25.80/hour or year, based on relevant years of experience. 

 

Employees working at locations operating 24/7 earn a differential.  Shift differentials are also available for evenings, nights, weekend and holidays.
