HEALTH PROGRAMS COORDINATOR

DEFINITION

Under direct supervision, plans, develops, coordinates, and implements a variety specialized public Health Programs and public information programs; conducts classes, workshops, educational activities, and other outreach activities; monitors effectiveness of assigned programs; assists families, providers, the community, and other interested parties with health information related to assigned programs; and performs related duties as assigned.

DISTINGUISHING CHARACTERISTICS

This single journey level class is responsible for planning, developing, coordinating, implementing, and overseeing assigned public Health Programs. Incumbents have responsibility for the day-to-day activities and functions of their assigned programs, which may include, but is not limited to Child Health and Disability Program, Childhood Lead Prevention Program, Communicable Disease Case Management, Dental Disease Prevention, Emergency Preparedness, HIV/AIDS Drug Assistance Program, Immunization Assistance Program, Maternal Child Health Program, Targeted Case Management Program, and/or Tobacco Control Education and Prevention, SNAP Ed Nutrition Program, or other assigned Health Programs. Incumbents will be assigned programs based on the needs of the department.

SUPERVISION RECEIVED AND EXERCISED

Receives direct supervision from assigned supervisory staff. Provides direct supervision to technical and administrative support staff, as assigned.

• Plans, develops, organizes, implements, and coordinates one or more assigned public Health Programs.
• Oversees and directs one or more specialized health services programs, which may include Child Health and Disability Program, Childhood Lead Prevention Program, Communicable Disease Case Management, Dental Disease Prevention, Emergency Preparedness, HIV/AIDS Drug Assistance Program, Immunization Assistance Program, Maternal Child Health Program, Targeted Case Management Program, and/or Tobacco Control Education and Prevention.
• Ensures that program goals and objectives are met.
• Oversees needs assessment activities and strategic planning.
• Develops outreach and marketing strategies, which includes the development and distribution of materials such as reports, pamphlets, exhibits, and news releases related to assigned programs.
• Assesses individual and community needs, including health education needs, related to assigned programs.
• Presents materials to groups, agencies, and individuals.
• Evaluates the effectiveness of health education components of assigned programs.
• Researches grants and develops proposals and completes project and grant evaluation requirements.
• Establishes and maintains liaisons between the Health Department and other public and private agencies, community organizations, professional groups, and other stakeholders.
• Provides consultation and guidance to individuals and community groups.
• Implements strategies to raise awareness of health issues, particularly those related to the assigned programs.
• Contacts community agencies and groups to promote assigned Health Programs.
• Develops, organizes, facilitates, and works with a variety of citizen advisory groups and/or coalitions.
• Plans and develops community events to promote assigned Health Programs.
• Prepares, coordinates, schedules, and conducts presentations before school groups, community organizations, youth groups, and other interested persons.
• Gathers, evaluates, and completes quarterly, annual, and final reports.
• Collaborates with surrounding counties and communities on issues related to assigned programs, health education, and/or other related functions.
• Participates in assessing community health needs and resources.
• Develops and monitors the budget for assigned function.
• Provides statistical analysis, surveys, and other services.
• Performs a variety of both routine and specialized clerical support work related to assigned programs.
• Depending on the needs of the department, County, and assigned programs, may collaborate and/or participate in emergency response operations of the County of Lake.
• May lead the work of staff on a project or day-to-day basis related to public health accreditation.
• May recommend policy and procedures related to assigned program(s), health education, and/or related functions.
• Performs related duties as assigned.

MINIMUM QUALIFICATIONS

Knowledge of:

• Principles of program planning, development, and implementation.
• Goals, functions, and activities of assigned public Health Programs.
• Principles, methods, techniques, and materials of public health education and outreach.
• Functions, programs, and services of public and private agencies involved in health education and outreach activities.
• Public health statistical and survey methods.
• Effective methods and techniques for information dissemination.
• Effective methods and techniques for working and communicating with at risk youth and adults, or special populations.
• General goals and purpose of public health theories, services, and education and outreach programs.
• Community resources and demography.
• Budget development and fiscal controls.
• Principles, practices, and methods of health education and outreach.
• Laws, rules, ordinances, and regulations impacting assigned public Health Programs.
• Principles of grant development, administration, and reporting.
• Proper English spelling, grammar, and punctuation.
• Customer service principles and techniques.
• Written and oral communications skills.
• Computers and software programs (e.g., Microsoft software applications) to conduct research, assess information, and/or prepare documentation.
• Principles and techniques for working with groups and fostering effective team interaction.

Ability to:

• Plan, develop, organize, and coordinate the services, functions, and activities of assigned public Health Programs.
• Develop health education and outreach programs for a variety of stakeholders.
• Oversee the development and administration of grants related to assigned public Health Programs.
• Read, interpret, and apply policies, procedures, and regulations.
• Coordinate activities of various groups related to the assigned public Health Programs.
• Communicate clearly and concisely, both orally and in writing.
• Regularly work well under pressure, meeting critical deadlines.
• Utilize a computer, relevant software applications, and/or other equipment as assigned to perform a variety of work tasks.
• Establish, maintain, and foster positive and effective working relationships with those contacted in the course of work.

Licensing and Certifications:

Possession of, or ability to obtain, an appropriate valid California Driver's License.

Education and Experience:

Bachelor's degree from an accredited four-year college or university with major coursework in public or community health education, health science, psychology, sociology, education, or a closely related field.

AND

Three (3) years of full-time experience coordinating and managing health education services and/or Health Programs and functions, and at least two (2) years in a supervisory capacity.

Additional directly related experience and/or education may be substituted.

Incumbents generally work partially in an office environment with moderate noise levels and controlled temperature conditions and partially in community environments. There may be occasional exposure to extreme temperatures, loud noise, fumes, noxious odors, dust, mist, gases, and poor ventilation; underground, confined, or restricted workspaces; and heights more than five stories above ground level. Incumbents may interact with upset staff and/or public and private representatives in interpreting and enforcing departmental policies and procedures

Positions in this class typically require: sitting, stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting, fingering, grasping, talking, hearing, seeing, and repetitive motions.

Sedentary Work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

Light Work: Exerting up to 25 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.

Positions in this class may require local and statewide travel as necessary.

A successful candidate will be required to submit to and pass a background check prior to the appointment. Disaster Service Workers: As members of the County of Lake Emergency Services Organization, all Lake County employees are designated as Disaster Service Workers during a proclaimed emergency and may be required to perform certain emergency services at the direction of the department and / or County.
