Your Specific Responsibilities:

The incumbent is part of a global public health organization and is expected to support the Bay County Health Department in its mission to protect, promote and improve the health of all people in Florida through integrated state, county and community efforts and its vision of being the Healthiest State in the Nation.  Furthermore the incumbent is expected to incorporate the agency values of Innovation, Collaboration, Accountability, Responsiveness and Excellence in all aspects of their work. The incumbent will come to work and exhibit an attitude of friendliness and eager customer service towards all clients and act respectfully and courteously to fellow employees and clients to provide excellent customer service.

HIV Reengagement/Linkage

Locate people living with HIV who are considered “lost-to-care” and reconnecting them with medical/support services.

Work with clinic staff and community partners to expand the outreach and engagement efforts. Identify barriers and assist the client navigate through the linkage to care process. Receive state linkage task assigned by the Regional Linkage Coordinator that require research and completion within 30 days.  Track and document state assignment contact attempts. State assignments require 1 attempt within 7 days of receiving the assignment. Contact Attempts can be made by letter, phone call, text message, or field visit. Minimum of 3 contact attempts must be made before closing the linkage record. Assist clients with telehealth visits both in office and in the field.

Ensure newly diagnosed clients are linked to care and support services within 30 days. Complete debriefings with the RLC which assess the effectiveness of each outreach activity. Assignment updates are sent to the RLC monthly to be compiled and reported to the HAP-C quarterly. Assist other prevention & DIS team members locate and contact hard to reach clients. Research multiple data bases to review clients that have previously been dispositioned “lost-to-care” or have not been seen by a provider in 18+ months.

Annually review clients in care with outside providers to ensure adherence. Utilize multiple software platforms to reference and compare data for accuracy. (Accurint, STARS, Careware, HMS, etc.) Update and maintain internal Data To Care tracking history on a monthly basis.

HIV Prevention

Administer rapid HIV & Hepatitis-C antibody test and conduct client centered pre- and post-test counseling including but not limited to risk reduction and referrals. Supports the Health Departments “Testing Tuesdays” in a rotating capacity offering rapid HIV & Hepatitis-C screenings and counseling. Participate in CHD community outreach events providing education, awareness, and rapid testing. Phlebotomy training to draw labs in the field, lab, or mobile medical unit. Maintain current knowledge of HIV/STD transmission and prevention information. In addition, maintain current adherence strategies and policies     

EARS and Staff Meetings

Completes all required reporting such as EARS, documentation requirements, requests and assignments correctly and within allotted time frames.  Attends staff meetings, in-service trainings and other professional and staff development activities.

Other Duties

Assists in additional office related activities and duties as assigned.

Qualifications:

Must have a valid Drivers License.

Phlebotomy skills.
