Fully remote Health Coach

Using a unique care model to help patients find the best path forward, Vori Health connects patients to a trained care team that includes a nonoperative physical medicine physician, a Health Coach navigator, and a physical therapist who manage the initial patient assessment and then work to coordinate all aspects of care. The Health Coach plays a vital role in working with our members to empower them to take ownership of their health by finding their internal motivation, implementing problem solving strategies, and achieving their wellness goals. Our Health Coaches also play an integral part in ensuring that we continually iterate to improve our product and overall user experience.
Our Health Coaches are our patients’ greatest advocates on their wellness journey by supporting them through their personalized care plan here at Vori Health. This is done through the implementation of modern behavioral change research and strategies to deliver sustainable life-changing health outcomes.
Vori Health coaches exude confidence in working with interdisciplinary care teams in designing optimal patient-centered care consistent with Vori Health’s standards. This role reports to the Director, Health Coaching.
What You will Do:
Essential Functions:
Consistent use of Vori Health’s behavior change solutions
Implement coaching techniques to improve patient outcomes
Provide video consultations and messaging with patients of Vori Health to improve their wellness goals
Design and support patient care plans through the utilization of motivational interviewing, SMART goals, coaching best practice and behavior change techniques
Proven aptitude for managing an ample patient caseload
Document interactions and interventions as directed by Vori health technology tools
Effectively collaborate and relationship building with Vori Health’s talented, cross-functional teams
Engaging on a regular cadence in a professional and collegial way with physicians, physical therapists, advanced practice providers, dieticians, and others
Troubleshoot unexpected issues so that patient needs are placed first and gaps in care are averted
Utilize critical thinking to discern and refine the correct provider for patient needs, stratifying the urgency of the referral amongst clinical team members as needed
Assist in the development of technology and service innovations to ensure a closed loop system of communication and coordination with our Vori Health providers and network specialists.
Provide feedback to colleagues regarding individual and team performance; be receptive to feedback on your performance
Communicate and collaborate with external Clinicians as appropriate
Maintain professional and technical knowledge through attending educational workshops, reviewing professional publications, etc.

Who You Are:
Required:
Board Certified Health and Wellness Coach
Proficiency with computers, virtual care, database management and data tracking
Knowledge of Motivational Interviewing, establishing SMART goals with patients, awareness of behavioral stages of change
1+ years experienced Health Coach and/or wellness educator, conducting individual health consults or group workshops
Demonstrated competence and passion for digital care delivery or telehealth: Comfort evaluating and treating patients remotely utilizing a variety of digital tools to manage care
Outstanding written and spoken communication skills across various channels and groups and managing team conflict
Proven record of matching patient needs to treatment plans and proven record of implementing these plans safely and effectively
Strong sense of empathy, a compassionate outlook, and the ability to motivate patients into pushing their limits in a responsible, safe way
Proficient with Microsoft Office Suite or related software
Reliable high speed internet connection
Dedicated home office space- private and free from distractions and noise.
Comfort working within a startup environment of rapid pace and constant change.
Excellent time management skills with a proven ability to meet deadlines
Nice-to-haves:
Bi-lingual, Spanish speaking
Experience in working toward specific outcomes in patient achievement, in the context of delivering interventions and within an integrated team
Understanding behavioral interventions and approaches to behavior change
Strong communication, proactive, growth mindset and collaborative working style
Proven record of accomplishment in creating exceptional experiences with patients
University degree in health science or social science discipline
Ability to troubleshoot unexpected issues so that patient needs are placed first and gaps in care are averted
Willingness to go the distance to provide support to our patients and fellow clinical care team
Ability to work in multiple technology platforms simultaneously, using professional and clinical judgment and critical thinking in accessing and documenting information that pertains to each patient’s referral process
Ability to maintain attention to detail while also seeing the big picture
Excellent interpersonal, organizational, and problem-solving skills
Ability to prioritize tasks and to delegate them when appropriate
Critical thinking skills, flexibility, and the ability to design innovative solutions in a fast-paced environment
Graceful at managing expectations while pushing for continuous improvement
Experience and comfort working closely with clinical teams to accomplish shared goals and drive positive collaboration
Commitment to be a member of a learning organization, open to development and continuous feedback
Work authorization/security clearance requirements:
Authorized or able to provide required documents to work in United States or Canada.
