Position Summary:
The Health Coach I is commissioned to ensure every patient that is referred to Health Care Services is properly registered within the EMR. The Health Coach I is responsible for verifying insurance eligibility, verifying demographics and all patient information. In addition, the Health Coach I will be responsible for scheduling visits for patients that consent to services within Health Care Services.
Working under the supervision of the CRNP and Health Coach Manager the Health Coach I will assist with the initial welcome call and education for new patients and then provide follow up calls as needed and coaching to help patients remain engaged in the program and ensure compliance with their plan of care.
All patient-related activities are documented and maintained in the patient chart. Health Coach I works in close coordination with the onboarding team members and Health Coach Leads to ensure timelines are met and patient engagement remains active.
Essential Duties/Responsibilities:
· Responsible for gathering all patient demographic information.
· Responsible for verifying patient primary and secondary insurance as well as ability to pay co pay if necessary.
· Verifies primary, secondary, and emergency contact information.
· Communicates eligibility changes or ineligibility immediately.
· Establishes relationship with team and works closely to onboard patients.
· Foster a trusting relationship with patients and engage patients in discussions regarding their own health care.
· Maintains positive demeanor and good customer service with patients and caregivers.
· Document all activity in a comprehensive, accurate and timely manner.
· Treats every event with the appropriate urgency and ensures all patient referrals to clinical team yield results.
· Perform patient check out process at the conclusion of visit to include processing of patient referrals and scheduling of follow-up and internal specialist visits.
Additional Duties:
Education and Experience Requirements:
· Medical Assistant certification or equivalent training and education required.
· AA in nursing or related field desired.
· A minimum of 1 years’ experience in health care services.
· Experience working with health systems, physicians’ practices etc. desired.
