Job Description
Position Summary:
The Health Coach will work directly with GrayHawk Health patients to holistically coordinate care. The Health Coach will provide direction, education and psycho-social support enabling patients to become advocates of their own health.
The Health Coach will partner with the care coordination team to ensure that the “GrayHawk Way” is lived out in every patient engagement.
Accountabilities/Criteria:
· Ensure all assigned patient charts are reviewed weekly
· Contacts patients and facilitates closing gaps in care
· Empathize, connect, and partner with patients to develop trust driving positive behavioral changes by leveraging various counseling techniques
· Uphold company values and maintain a strong team dynamic including sharing your specific expertise with your teammates.
· Learn and incorporate the integration of clinical and engagement training for Care Plan Management
· Leverage therapeutic techniques empathize, connect, and partner with patients developing increased outcomes in self-management, self-efficacy, and engagement
· Engages the patients to assess current state, collaboratively set goals, and review the post-engagement summary
· Instructs the patient on condition management, behavioral support, and addresses emotional demands of having a chronic condition
· Reinforces the clinical care plan and follows up for ongoing engagement
· Assist in training new Health Coaches with care plan development (what is it, purpose of it, how do we work a completed care plan)
· Successfully achieve key performance indicators.
· Assist new Health Coaches with updating or changing care plans to support each individual patient
· Consistently review and consult with Health Coach team to better understand the full patient experience
· Stay up to date on clinical standards for chronic disease management
· Understands and adhere to program requirements for CCM
· Document all activity in a comprehensive, accurate and timely manner
Education and Experience:
· PTA, OTA, or MA required
· Proficient in medical documentation
· 1-2 years’ experience in case management/care coordination preferred
