Position Title: Health Coach
Position Type: 6 Month Grant Funded Full Time; 35 Hours/Weekly; Per Diem Work an option

Responsibilities:

• Conduct initial consultations, including individual evaluations for new consumers
• Develop customized health care plans addressing client specific needs
• Provide consumers with education regarding substance use disorders, behavioral health, physical wellness and self-management
• Educate consumers in various treatments related to hepatitis, HIV, and STIs as appropriate
• Develop and implement self-monitoring tools for consumers in OUD, infectious disease, and co-morbid illness management
• Create a survey for clients and/or family members to complete after a Health Coaching session

Requirements

• Health Coach Certification approved by the National Board for Health and Wellness Coaching (NBHWC)
• Previous experience working with clients living with chronic medical and behavioral health conditions
• Maintains relevant knowledge regarding health care developments
