Serve as an integral part of an outreach team, assisting in patient care management and care transitions.
Reaching out to patients with chronic care needs to ensure their understanding,
agreement and participation in wellness.
Screen and assist patients with connecting to food, housing and community resources.
Help patients gain the knowledge, skills, tools and confidence to become active participants in
in their care.
Coach patients to schedule their own healthcare appointments and transportation.
Bridge the gap between clinician and patient, by providing individual support and
assistance in navigating the health care system to provide continuity of care.
Experience working in a healthcare office setting, or community outreach and education required.
Knowledge of health promotion, nutrition, and counseling preferred (specific disease and lifestyle related topics ideal).
Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.
Ability to take direction, follows through with tasks, and work independently.
Flexibility, optimism, and a cooperative working style.
High School Diploma or Equivalent; clinical training (i.e. MA, CNA, LPN) a plus.
Fluency in Spanish preferred.
Competent in basic computer skills; knowledge of using an electronic health records system preferred.
