Form of workHealth Coach assists patients in finding innovative ways to inspire healthier habits. This position will work closely with chronic disease management team to overcome patient barriers to care. Will employ a patient centered approach to encouraging incremental lifestyle changes that are conducive to improvements in overall health. Will utilize therapeutic communication techniques such as reflective listening and motivational interviewing to motivate change.
Qualifications
- High School diploma, 2 years of college preferred.
- Excellent communication skills.
- Ability to address needs of culturally and demographically diverse group of patients with chronic disease.
- Must be able to work successfully in a team environment.
Conditions of Employment
