Reports to: Women’s Empowerment Director
- Position Summary: The goal of the GO PLACES Project is to introduce and prepare justice-involved youth and young adults ages 18-24 for the world of work through placement into paid work experiences, and on a path to more equitable career opportunities with their peers through continued mentorship and involvement. With the goals to:
- Decrease recidivism rates within the target population.
- Decrease violent crime rates within the target areas.
- Increase labor participation rates within the target population.

Duties and Responsibilities:
- Provide support to the YWCA / GO PLACES team for enrolled participants during specified shift hours by facilitating wrap around services including participant welfare checks, benchmark completion of goals, making appointments with/for clients, completing client referrals, offering client transportation when needed by the outreach team and offering mentorship through your lived experience.
- Work with the GO PLACES Team and collaborate with other community partners to identify the community or neighborhoods with a high need for low-barrier violence prevention, enhanced employment skills and safe access to resources. Develop a workflow and cultivate networks with community members to establish the needs in the community and ways to address those needs. Brainstorms with the GO PLACES Team to help have those needs met. If the GO PLACES Team cannot help, the Community Health Worker will work to identify other organizations that may be able to support.
- Use appropriate documentation via The Department of Labor / GO PLACES platforms to navigate case management and record data for outcomes.
- Develop relationships in identified communities to facilitate low barrier care to addressing violence prevention and employment needs, mental health needs, or general support/resources. Help facilitate these services and arrange outreach needs as a trusted community member.
- Be a liaison to other community peer support workers, Community Health Workers, judicial organizations and health care organizations trying to provide care in overlapping locations or communities.
- Collaborate to organize outreach efforts and education events with stakeholders or community members including recruitment and training opportunities.
- Assists GO PLACES Coordinator and the YWCA with client case management work. (brainstorming/working together on solutions for clients or follow up care when needed)
- Utilize individuals with strong ties to the education system who have the ability to profoundly impact the messages that youth and young adult population within our communities are receiving through personal and professional lives.
- Support participants in overcoming a variety of systematic, mental, physical, and substance misuse obstacles impacting their lives by aiding them, their families, schools, and post high school programs by providing resources, education, and increased access to healthcare and resources for other stressors including housing, trauma, abuse, incarceration, and undocumented immigration.
Additional Job Functions and Working Conditions
- Complete a variety of weekly reports for analysis, follow-up, and submission for project reporting.
- Make community and home visits to participants to perform duties as a YWCA / GO PLACES employee.
- Performs other related work as assigned by supervisory and / or GO PLACES personnel.

- Experience in a Community Health Worker role or substantial relevant life experience.
- A willingness to gain a state recognized Community Health Worker certification within 6 months of employment if not currently certified (but has required life experience).
- Possession of a legal and current valid State Driver’s License.

- Maintain knowledge about NC Works resources, including but not limited to trainings and WIOA.
- Effectively connect with the community and facilitate client connections.
- Properly document outreach interactions, activities, and observations.
- Have the ability to engage individuals and move them into action by providing clear and accurate resource information.
- Ability to build relationships with respect to diversity, using active listening, casual counseling, and encouragement.
- Ability to make observations of community contexts, determine relevance to client success and document appropriately.
- Ability to carry out an effective home visit and/or community event with respect to personal safety, safety of client, professional boundaries, and time/ conflict management.
- Ability to inquire for follow-up data regarding successful client progress and/or continued barriers.
- Communication:
- Utilizes communication skills grounded within the context of individual and community culture.
- Ability to read a variety of reports, correspondence, ledgers, logs, applications, etc.
- Ability to prepare correspondence, reports, forms, audits, etc., using prescribed formats and conforming to all rules of punctuation, grammar, diction, and style.
- Ability to communicate to people with poise, control, and confidence.
HOURS OF WORK: Monday - Fridays, occasional evenings and weekends
Pay: $35k-$45k
