This position aids the Care Management team by working collaboratively to support client care plans and needs. Serving as an advocate for clients and functioning as a part of the care management team. The position will work in tandem with care managers, healthcare providers and community-based organizations to improve outcomes for the clients they serve. Working within their scope of practice, this position coordinates between health care services, recognizing the holistic needs of the client, inclusive of client specific social, cultural, and physical, oral, and mental health dynamics. This position will support programmatic goals and objectives in meeting performance improvement targets and supporting team development. The CHW performs para-professional work providing services to clients in the Health Department, and related work as apparent or assigned. Work is performed under the the moderate supervision of the Human Services Coordinator and Social Work Supervisor.

Position Responsibilities
Engages with target population to identify and address barriers that impede health outcomes.
Knows and recognizes social determinants of health and maternal and child health topics that impact clients and communities, to be able to give support and provide information on these topics.
Supports clients and communities through building new skills and promoting confidence in their own health while implementing interventions per the established care plan.
Processes service referrals from care team members appropriately, accurately, and timely according to established workflows.
Schedules and completes client encounters, home, office, and community as applicable.
Schedules and verifies health care related appointments as needed.
Serves as an advocate on behalf of the client among community services, primary providers, specialists, and other care team members to coordinate services.
Provides educational information to the care team, client, family, and caregivers, about community-based organizations within service area.
Documents services that are provided and submits required reports to supervisors.
Participates in multidisciplinary initiatives to improve efficiency and effectiveness of client health outcomes.
Adheres to agency policies and procedures, inclusive of HIPAA regulations and privacy and security policies.
Attends agency meetings, local and regional training, or other events, as required.
Provide outreach to clients and communities about services that are available and encourage enrollment of those services.
Willingly performs other duties, as assigned.
Domain 1: Data Analytics and Assessment
Collect, analyze, and manage the information gathered during the screening process to determine the member's needs and level of service to be provided.
Identify and describe factors that affect the health of the target population, along with resources for improving health in a community.
Access, collect and prepare quantitative and qualitative data for evaluation using various methods and tools to monitor outcomes and positively impact performance measures.
Domain 2: Policy Development and Program Planning Skills
Contribute to the implementation and evaluation of the Community Health Worker Program.
Applies program strategies, connects individuals to program, and monitors goals.
Engage in program strategic planning and monitors trends impacting implementation.
Domain 3: Communication Skills
Facilitate communication among individuals, groups, and organizations, internally and externally.
Assess the literacy of internal and external audiences and identifies the communication needs and preferences to guide and support members in achieving patient-centered goals with individualized interventions that move the member toward optimal health and wellbeing.
Disseminate and educate members on community resources to positively influence behavior and improve health while encouraging self-management through materials that meet literacy standards.
Domain 4: Health Equity Skills
Apply principles of ethics, diversity, equity, inclusion, and justice.
Engage in continuous self-reflection and identify how biases influence policies, programs, and services. Recognize the diversity of individuals and populations.
Identifies and communicates barriers that perpetuate health inequities.

Domain 5: Community Partnership Skills
Describe conditions, systems, and policies affecting Community Health and resilience.
Establishes and maintains relationships to improve Community Health and resilience.
Collaborates with community members and organizations.

Domain 6: Public Health Sciences
Identify public health programs and community organizations with authority to address specific health needs of the target population.
Apply public health sciences in delivering the 10 Essential Public Health Services.
Advocate for patient health and well being by recognizing the diversity of individuals and involving the individual in the decision-making process through a patient-centered & collaborative partnership.
Coordinate care across care team members and maintain regular communication to improve member's care and ensure public health services are provided in a coordinated effort that does not duplicate services.
Contribute to the development of organizational strategic plans for continuous improvement.
Qualifications
Basic knowledge of the social, economic, and environmental conditions of the target population to be served; working knowledge of basic maternal and child health with the ability to teach others; skill in basic reading, writing and simple mathematics; ability to communicate clearly and succinctly, verbal and written forms ability to exercise good judgement in appraising situations and ability to verbally communicate findings; Knowledge of governmental and private organizations and community resources; Knowledge of laws, regulations and policies which govern the program; Skill in establishing rapport and conducting interactions with individuals of all socioeconomic backgrounds, cultures and levels of authority. Must be able to function as part of a team. Requires the ability to understand and maintain confidentiality of verbal and written information; and how to obtain consent to share appropriately; requires organizational, problem solving and critical thinking skills; must be able to work with changing priorities; Ability to express ideas clearly and concisely and to execute work; Ability to operate a personal computer, standard office productivity software and proficiency with the technologies required to perform position functions; maintains a high level of professionalism.
Education, Experience, and Special Requirements
Community Health Workers meeting competencies is defined as having the following qualifications:
High School graduate or GED.
Required NC Association of Community Health Workers Standardized Core Competencies Training and certification with first 9 months of employment and a Valid NC Driver's License and auto insurance.
CHW should have lived experience of and immersed in community to be served; knowledge of social determinants of health, especially those specifically impacting community to be served and experience in public health or other related human service of healthcare setting/experience.
Special Requirements:
Valid driver's license.
NC Association of Community Health Workers Standardized Core Competencies Training and Certification
