Job description

This is a temporary contract position. Employee will serve as the Community Health Worker (CHW) for Alamance County Health Department Maternal Health Services. The purpose of this position is to work with individuals of reproductive age, their children and families to improve maternal and infant birth outcomes as well as health service delivery within the community.

The CHW is a trusted member of the community with an established close relationship and understanding of the community served. Duties for the CHW position include:


  • Advocate on behalf of the client and the nearby community to ensure clients receiving services to improve maternal and infant health.
  • Serve as cultural liaison between the client and the human services system.
  • Serve as Health Navigator - Facilitate access linking clients to services within the health department - WIC, Medicaid, reproductive life planning, preconception and interconception health, postpartum home visiting program, care management for high risk pregnancy and children, and other services (i.e. Transportation assistance) identified through NCCARE 360 and community partners.
  • Educate clients on assistance they can provide (12-month postpartum Medicaid coverage, educational information on smoking cessation, safe sleep, immunization schedule, injury prevention, and proper car seat use.)
  • Facilitate and provide NC Medicaid applications to clients.
  • Work closely with Maternal Health staff and care management team to determine client's eligibility.
  • Conduct Home Visits and or in-clinic follow ups.
  • Maintain a participant log and make proper notation in EPIC system.
  • Make referrals based on participant need for resources.
  • Maintain confidentiality of program participant information and associated records.
  • Collect data on program participants and services delivered.
  • Complete and submit reports as required by the grant.
  • Participate in program evaluation, participant satisfaction survey and activities as required by the State.
  • Participate in program webinars and meetings.
  • Register and successfully complete a NC Community Health Worker Association sponsored training within 9 months of hire.

This position will also be required to staff emergency shelters, assist with Public Health outbreak investigations and response, and perform additional duties as required by the Nursing Supervisor, Nursing Director, Medical Director and Health Director.

EMPLOYMENT INFORMATION: This is a grant-funded position which will begin February 1 2024. Selected candidate will not be an employee of Alamance County; he or she will be employed by the North Carolina Alliance of Public Health Agencies (NCAPHA) and contracted to work with Alamance County Health Department.

ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIES:

Considerable knowledge of the principles and practices of Community Health and outreach activities. Must be able to complete NC Community Health Worker Association training within nine months of hire.


MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

Graduation from a four-year college or university with a major in health education; or graduation from a four-year college or university and completion of one year of on-the-job training as a public health educator.

NECESSARY SPECIAL QUALIFICATIONS: Valid N.C. driver's license and have access to a personal vehicle.


PREFERRED QUALIFICATIONS: Knowledge and experience with Maternal-Infant Health. Fluent in Spanish and English both oral and written is preferred but not required.
