Full Time Registered Nurse (RN) - Home Health - Salem, OR

There is an open Full Time Registered Nurse (RN) Home Healthjob in Salem, OR. The Full Time Registered Nurse (RN) will be responsible for coordinating care for in-home, assisted living and senior housing residents. The Registered Nurse (RN) Home Health will conduct monthly assessments to review the resident's status and assist in achieving progress towards goals. Depending on acuity the RN will perform 5-7 assessments per day.Theassessment will be conducted on Medicare Advantage plan members residing in homes, assisted living and senior housing facilities in Salem, OR and the surrounding Marion county area.Marion County, ORincludes locations like Keizer, Woodburn, and Hayesville. Candidates could be scheduled in any one of these locations during an individual shift.

Responsibilities:

• Conduct routine monthlyassessments for a Medicare Advantage health plan
• Evaluate prior and current medical history, medical conditions and medication history
• Provide education on medical conditions and medications
• Work closely with the NP to coordinate care and ensure resident's needs are met
• Perform resident rounds and assist with advanced care planning

Work Hours:

• Monday and Friday
• 8:00 AM - 5:00 PM

Preferred Experience:

• 1 year of current RNexperience is preferred
• Care Coordination, Case Management or long Term Care experience is preferred

Preferred Qualifications:

• Registered Nurse (RN) License in OR
• Current CPR certification (BLS)
• COVID vaccination required

Education Requirements:

• ASN or BSN degree from an accredited school of nursing
