Multiple Case Planner vacancies available in Brooklyn, Harlem, and The Bronx
About the Position:
- Creates, in partnership with families, service plans that address safety, behavioral, educational, psychological, and permanency needs.
- Monitors safety, well-being, and progress against other critical outcomes by facilitating regular casework contacts and by documenting the same in progress notes and other case documentation.
- Collaborates with internal and external service providers and other stakeholders through clear and timely verbal and written communication.
- Participates actively in Family Court Hearings, Family Team Conferences, and other meetings to advance permanency for children and families.
Qualifications:
- Bachelor’s or Master’s Degree in Social Work;
- Bilingual Spanish speaking required;
- Critical Competencies:
- Detail-oriented;
- Excellent verbal and written communication skills
Flexibility, professionalism - Ability to function as a team player and possesses a demonstrated passion for advancing the work of serving children and families
- Ability to work with diverse populations
- Familiarity with NYS Connections system a plus
Position Type:
- Full-time position; salary commensurate with experience, in accordance with qualifications.
