SalaryThe Adoption Support Worker provides and supports adoption services by working to provide safe, secure care for children until they can be adopted or another permanency plan can be implemented and through a year of aftercare.
Develops and maintains good working relationships with DCF, JJA, and other community agencies
Completes or gathers written documentation for children’s files
Connects children and families to support services and resources needed
Provides direct in-home services including but not limited to parenting skills, life skills, child development, and behavior management.
Support case plan goals through teaching, role modeling, and demonstration
Participates in case conferences, family meetings, client assessments, and in the team-decision making process
Supervise and transport children and families as needed
Participates in team meetings, sharing pertinent information regarding recruitment and retention of perspective adoptive families
Reports unusual incidents through appropriate Risk Management, clinical and safety channels
Ensures clients’ rights are protected
Is knowledgeable of and follows all safety procedures
Implements SFM policies and follows directives as required. Follows and adheres to all pertinent SFM Standard Operating Procedures (SOPs), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms

Pay: $15.21 - $20.57 per hour
