his position focuses on ensuring that appropriate services and treatment are provided to clients in order to see them move towards permanency and experience successful outcomes. This is accomplished through a trauma-informed care lens using a faith-based perspective that recognizes the significant needs of the clients in the program, and views the TFC parents as the primary treatment provider. Advocacy and support are provided for the client and the TFC parent in a variety of environments including but not limited to community, school, work, and home. A strong team approach is utilized to ensure success.
Ongoing Care and Support
Family Support
- Coordinate the development of quarterly treatment plans to identify and evaluate strengths and challenges
- Participate in meetings:
- Family Support Team (FST) meetings to inform all of childs progress
- Education Plan (IEP) meetings to ensure appropriate educational services
- Participate in court hearings to support the families as needed
- Conduct weekly visits with TFC parents and child
- Conduct ongoing assessment of childs safety, behaviors and progress on treatment goals
Parenting Support
- Assess family strengths and needs
- Build rapport with TFC parent to provide support and encouragement, assess safety, and identify current needs
- Assist TFC parents in implementing the principles of the trauma-informed model that agency utilizes
- Provide 24 hour/7day on call support to TFC parents for crisis management (directly or through a rotating on-call schedule)
- Assist foster parents in finding respite services to prevent burn-out in both emergency and non-emergency situations
- Review and update Treatment Plans every 90 days in order to report progress and address concerns Maintain regular contact with case manager to ensure consistent and appropriate care of client
Qualifications:
- Assurance of a Christ-centered life
- Bachelors degree in social work or a related field
- Two (2) years related experience
- Ability to travel throughout the state with occasional overnight travel
- Flexibility in work schedule including evenings and weekends when necessary
