Family Planning Patient Coordinator

Position Summary:
The Patient Coordinator will help to ensure timely, compassionate care for patients seeking abortions, birth control, and other Family Planning services. The primary responsibilities of this role include triaging the Family Planning Mobile Line, performing initial patient intake, options counseling, and scheduling appointments. The Patient Coordinator will work closely with the Family Planning Program Manager to reduce barriers for patients and streamline referrals.
Location: Remote within Western New York, occasional attendance at monthly division meetings preferred
Time Commitment: Approximately 16 hours per week, flexible timing but must be within 8am-5pm EST, Coordinator and Program Manager will determine weekly schedule. May be asked to assist when Program Manager is out of the office.
Compensation: $25-30 per hour
Roles and Responsibilities
? Answer incoming calls and texts to Family Planning Mobile Line
? Perform initial intake, including medical history, for new patients
? Register patients in practice management system and generate intake notes in electronic medical records
? Counsel patients on available abortion options and assist in decision-making
? Triage incoming sick calls for providers
? Schedule in-person and telehealth appointments
? Assist patients in navigating barriers to care such as transportation, escort requirements, time off work, etc.
? Attend monthly Family Planning Division meetings
Supervision: This role will be supervised by the Family Planning Program Manager. Monthly check-ins will be incorporated into Coordinator?s schedule.
Qualifications and Profile:
? Experience working in abortion care provision or with people seeking abortions
? Experience with practice management systems and electronic medical records
? Demonstrated commitment to reproductive justice and expanding options in WNY
? Has an appreciation of how racial injustice intersects with reproductive justice
? Queer-positive
? Ability to work independently while maintaining open and effective team communication
? Ability to prioritize efficiently
? Highly organized and exceptional attention to detail
? Commitment and sensitivity to confidentiality
? Has personal cell phone that can be used with Hover Mobile Application to receive incoming Family Planning calls, a laptop will be provided.
