The Family Partner serves as a parent, guardian, or caregiver of a child with emotional, behavioral or mental health needs, who has experience navigating the behavioral health services necessary to achieve a wellness and recovery-oriented lifestyle. Overall, the Family Partner contributes to the CHS high performance culture by exhibiting our values and providing quality results that position CHS as the leader in delivering proactive behavioral health, case management, community and early childhood solutions for children and families.

Primary Job Functions

1.  Support and coordinate services for caregivers and families requiring diversion and prevention support.

  • Facilitate and participate in Family Team Meetings with the Wraparound Specialist to support and advocate for family perspective and choice.
  • Coordinate with Wraparound Specialist and family team to assess family strengths and identify their needs to achieve goals and family vision.
  • Attend team meetings, coaching, supervision, training, and quarterly reviews as scheduled to obtain updated community resources, identify community needs, and buiFld skills.
  • Foster and maintain ongoing relationships with youth, family members, providers, and community resources in order to coordinate services and advocate for families.
  • Liaise with service providers, the education system, the Department of Children and Families, and Substance Abuse and Mental Health (SAMH) in order to ensure services are culturally and linguistically competent.
  • Meet directly with families to assess family strengths and needs to achieve goals and family vision.
  • Maintain meaningful involvement with the parents/caregivers and their child’s planning process.
  • Attend and participate in local community meetings e.g. Federation of Families and Wrap Café.
  • Empower families and youth to articulate their needs and concerns effectively.
  • Maintain confidentiality and adherence to HIPAA in all oral and written communications.
  • Develop and maintain client records and program documentation according to contract and CHS standards.
  • Provide transportation for clients when deemed appropriate to support the plan of care.

2.  Contribute to a positive, engaging work environment.

  • Develop a strong knowledge base and stay current on job-related issues and trends.
  • Participate actively in departmental meetings, training and education, as well as the quality process.
  • Comply with CHS’s code of conduct, policies, procedures and other obligations.
  • Assist with training other team members and providing back up when necessary.
  • Pick up projects on the fly; perform other duties as assigned from time to time.
  • Demonstrate the CHS Common Bond values in the performance of all job duties.

 

Job Qualifications

Education, Licenses & Certifications:

  • High School diploma or GED equivalent, required.
  • Associates degree from an accredited university, preferred.
  • Florida Driver’s License within 30 days from hire with daily access to a reliable and insured vehicle, required.
  • Successful completion of Certified Recovery Peer Specialist certification within 12 months of hire, required.
  • Certified Recovery Peer Specialist through the Florida Certification Board, preferred.

Experience:

  • Experience as a primary caregiver of a child with a mental/behavioral health disorder (adoptive, foster, or biological), required.
  • Experience as a consumer of family services in order to successfully advocate for families and provide resource linkages, required.
  • Experience navigating the behavioral health/child welfare/juvenile justice systems necessary in order to achieve a wellness and recovery-oriented lifestyle, required.
  • Peer-to-peer work or volunteer experience, preferred.
