Duties and Responsibilities
- Be a positive ambassador and champion with members for the Collaborative Care program.
- Engage in initial and continuous outreach to a cohort of members on a regular basis (as assigned) virtually and in person, to enroll them in the program.
- Ensure timely documentation of the enrollment in the Clinical EHR.
- Provide appropriate education and answer questions about the program, service expectations, and what to expect working with the behavioral health clinician and the broader team.
- Complete initial member screenings to determine enrollment eligibility.
- After enrollment, ensure a warm hand-off to the behavioral health clinician or schedule an initial appointment for enrollment.
- Research and develop community outreach and resources to ensure the team is able to meet enrollment targets.
- Assist with coordination of and participation in community engagement events, as needed to fill cohorts.
- Coordinate and complete effective outbound calls, to hit individual enrollment targets and ensure the team meets enrollment goals.
- Prepare and maintain records, reports, paperwork, and members’ diagnostic records, maintaining the confidentiality of members at all times.
- Accurately complete all case notes in a professional, timely manner, producing high-quality documentation that enables members to receive the best treatment possible.
- Show each member respect and compassion without crossing therapeutic boundaries, infringing on ethical standards.
- Actively participates in process improvement by identifying areas of inefficiency and contributes to opportunities for enhancement.
- Executes additional responsibilities as required and/or designated by Janus Healthcare management.
- High School Diploma / GED or higher.
- 2+ years in healthcare customer service
- Excellent interpersonal skills
- Ability to quickly build rapport with prospective members over the phone
- Experience working with high-risk, complex adults preferred.
- Capable of effectively overcoming objections and conveying benefits of enrollment
- Positive attitude and empathy for patient needs
- Strong organizational, data entry, and database management required
- Attention to detail and ability to follow proper processes and procedures with care
- Ability to simplify information and communicate effectively, verbal and written

- Primary remote – but able to work Eastern time
- Full-time, hourly, 40 hours/weekly
- $17.50 - $20.00 per hour depending on experience
