Position Summary:

At Roots Community Health Center and Dream Youth Clinic we believe in youth-led, adult-guided collaboration and are committed to "meeting youth where they are," and assisting them along their journey to wellness by imploring principles of Positive Youth Development, belonging, and youth agency. The Youth Health Navigator position works therapeutically with highly vulnerable youth populations and is able to take initiative, multi-task, communicate effectively, collaborate and advocate for the needs of youth clients.

The Youth Health Navigator will provide health navigation to Dream Youth Clinics clients at-risk-of or involved in commercial sexual exploitation (CSEC) also known as sex trafficking.

Duties and Responsibilities:

  • Provide Roots health navigation services for the Dream Youth Clinic CSEC youth population.
  • Participate in collaborative case conferences with community partners and city agencies working with CSEC.
  • Work with the Adolescent Services Department Administrator and Program Coordinator to organize and maintain clinical data on youth affected by CSEC at the Dream clinic sites.
  • Support Dream youth clinic staff in working with CSEC youth, including but not limited to, collaborative client management and co-facilitation of clinic supportive groups. Other supportive CSEC client youth services as identified.
  • Serve as the primary case manager for youth patients/clients, facilitating initial introduction to programs/services, and maintaining ongoing contact with each assigned youth patient/client.
  • Carry a caseload of 10-15 patients, depending on case management model and acuity.
  • Develop a long-term relationship with the client/patient that engenders trust.
  • Make patient outreach calls based on patient needs, and provide the patient with her/his business cell phone number for ongoing access.
  • Engage patients in a comprehensive barrier assessment to address the following barriers: psychosocial health, criminal history, physical health, probation/parole status, mental health, job readiness, substance use, DMV/driver license issues, family/support system, employment/training history, counseling needs, child support orders/custody, IRS/FTB liens/levies, access to food, clothing and hygiene items, civil restitution orders, access to healthcare services.
  • Enroll clients in public benefit programs (ie: Medi-Cal, CalFresh) when applicable.
  • Assist with logistics pertaining to medical and social services.
  • Provide health coaching based on what the patient/clinician has identified as their health concern (glucometer training, etc.).
  • Support medical providers and clinical staff with documentation of patient information.
  • Participate as part of the Healthy Measures team in weekly case management meetings and other meetings as required.
  • Document encounters in the electronic health record (EHR).
  • Participate in trainings and conferences to build skills.

Competencies:

  • Community Health Outreach Worker Certification or equivalent.
  • Bachelor’s degree with 3 years’ experience performing social services functions or Associate Degree in related fields with 4 years’ experience working in social services preferred.
  • 2 years’ experience working in treatment programs preferred.
  • Experience working with marginalized youth populations locally, including communities of color, preferred.
  • Experience working in non-profit organizations, or a community clinic preferred.
  • Cultural congruence with the Dream Youth Clinic patient population.
  • Enjoy working with diverse youth (cultural, linguistic, and gender diversity).
  • Ability to incorporate creativity and forward thinking in working with youth.
  • Trained/willing to receive training in health coaching.
  • Excellent communication skills.
  • Demonstrated ability to work collaboratively and in team settings.
  • Interest/proficiency in working with urban underserved populations, individuals recently released from incarceration, involved in the foster care system, or affected by commercial sexual exploitation (CSEC).
  • Must be able to work on-site Monday – Friday and may include occasional weekend event duties.

Physical Conditions:

  • Frequent standing, stair climbing, bending, stooping, lifting (25 lbs. max).
  • Gross and fine manual dexterity.
  • Ability to visually observe staff to understand and interpret non-verbal behavior.
  • Ability to operate standard office machines and equipment including keyboards.
