Job Description
Under the supervision of the Associate Director of Communicable Disease and Reproductive Services. Prevention Navigator is responsible for all aspects of a substance abuse, mental health, and HIV prevention program with emphasis in the LGBTQ+ community ages 18-29 in South Los Angeles.
Education & Experience
- Bachelor’s degree in related field (human services, health services, or a similar field).
- Comfortable working with diverse populations including formerly incarcerated individuals, undocumented individuals, LGBT populations, communities of color, and those experiencing homelessness, substance abuse, and/or mental illness;
- Two years’ related experience in a health or social service setting preferred related field;
- At least 2 years’ experience conducting community and/or street outreach, specifically to hard–to- - reach populations
- Experience with community organizing • Excellent oral/written communication, interpersonal and oral presentation skills.
- Bilingual – written and speech Spanish/English (required).
- Strong computer skills
RESPONSIBILITIES
- Responsible for the overall implementation of a youth program targeting Black/Latinx LGBTQ+ young adults ages 18-29.
- Provide case management services to patients in need of substance abuse, mental health. HIV and sexually transmitted infections (STI)
- Screen patients, service planning and resource identification; linking clients to needed services
- Work with interdisciplinary team to provide comprehensive SA, Mental Health and HIV prevention, sexually transmitted infection (STI) education and services.
- Conduct patient, identification, assessments, diagnosis, care plans and interventions
- Conduct service implementation and coordination, monitoring service delivery, and advocacy
- Conduct presentations and trainings to staff, patients, and community members in a variety of settings
- Establish and develop working relationships with clients; initiate outreach to the target population using telephone, mail, and visits in the community.
- Attend regularly scheduled meetings and maintain communication with program director and other staff members.
- Maintain outreach activity calendar/Log (i.e., location, length of outreach event, encounters per event).
- Assist with program evaluations and updates to program and services.
- Attend appropriate community resource meetings and trainings assigned.
- Create and implement an evaluation tool to measure the success of counseling and outreach efforts.
- Conduct traditional outreach activities and classes in the community and non-traditional outreach in social media. • Implements Peer Prevention Leader @ SJCH
- Facilitate Skills Building workshops to prevent Substance Abuse, HIV and Mental Health
- Provide PrEP, PEP, STI and other risk-reduction education to patients at risk for HIV
- Conduct presentations and trainings to staff, patients, and community members in a variety of settings
- Link patients to other services as needed (PrEP and PEP, HIV and VH Testing) • Some evenings and weekends required
- Other duties may be assigned or may be modified as business needs dictate
This position is grant funded. The grant is scheduled to expire on 09/29/2027. The grant may or may not be funded again after that date.
