Position Description
Under the supervision of the Medical Director, manages the care of participants to provide high quality, cost effective care. Coordinates services with primary care team. Triages and assesses participants as needed. Acts as a gatekeeper for participants in all settings and levels of care. Provides participant, caregiver, family and staff teaching and communicates participant's condition to caregivers, primary care, and the interdisciplinary team. Acts as a liaison for effective communication between the LIFE center and contract facilities. Coordinates care with the Interdisciplinary Team and contracted facilities to provide services, as outlined in the Plan of Care. Coordinates end of life care so participants receive the appropriate medical, pharmaceutical, and psychosocial services.
As a multisite provider of PACE community-based services for seniors in Cumberland, Gloucester, and Salem Counties, an Inspira LIFE employee may work at either or both the Vineland or Williamstown LIFE Program as census changes or operational needs occur to meet the needs of our participants.
Education & Experience:
•Graduate of a school of professional nursing.
•BSN preferred.
•Palliative care experience preferred.
•Case management experience preferred.
•Minimum of 1 year working with the frail or elderly.
•2 years of Nursing Experience.
Certification/Licensure:
•Current BLS (American Heart Association).
•Certification in appropriate specialty preferred.

Position Responsibilities
•Coordinates all transitions of care between care settings and from one level of care to another for LIFE participants, maintaining timely communication with all parties.
•Performs nursing duties as required or requested by DON for coverage of clinic or home care nursing functions.
•Assists in the nursing treatment, monitoring and testing of participants as needed.
•Observes, records and reports participant's condition and reaction to drugs and treatments to physicians.
•Notification of any marked change in the participant's condition to the IDT Team.
•Provision of emergency care including arrangements for transportation.
•Acts only within the scope of his or her authority to practice.
•Demonstrates positive interpersonal relations in dealing with fellow employees, supervisors, and other department employees to maximize positive employee relations and productivity.
•Demonstrates professional practice in patient care by use of established standards in planning, organizing, implementing and evaluating.
•Maintains current, written records in compliance with regulatory standards; prepare a variety of written reports and correspondence.
•Demonstrates understanding of participants as unique individuals in need of loving, supportive, emotional, physical and spiritual care.
• Perform duties in accordance with Code of Professional Nurses.
• Acts only within the scope of his or her authority to practice.
•Demonstrates the ability to obtain information and interpret information in terms of patients' needs.
•Demonstrates knowledge of growth and development.
•Exhibits appropriate communication skills necessary for ages served (adult/geriatric).
•Proactively involves the healthcare team in the care of the participant.
•Facilitates the Palliative Care Team/Committee.
•Collaborates with providers to assist participants/caregivers in creating and understanding goals of care.
•Works with the Interdisciplinary team to provide end of life care which includes pain and symptom management and culturally sensitive practices.
•Assists through and/or teaches participants and family the death and dying process and supports ethical decision-making.
•Acts as nursing home liaison, which includes but not limited to, communicating with staff at contracted facilities to improve communication and coordination of care.
•Facilitates joint plan of care sessions between contracted facilities and LIFE for all LIFE participants who reside in contracted facilities, at a minimum of every 6 months or when there is a significant change in status.
•Acts as a hospital liaison, which includes but not limited to, communicating with staff at in network and out of network hospitals to improve communication and coordination of care.
•Performs other duties as required or requested.
