Job description

Recovery Centers of America's South Amboy, New Jersey location is currently seeking a full-time Nurse Case Manager.

Schedule: Monday-Friday 8:30am-5:00pm

Position Overview:

The Nurse Case Manager will operate as a member of the multidisciplinary treatment team by working closely with the clinical team. The Nurse Case Manager will assist in identifying and developing individualized Medicated Assisted Treatment (MAT) plans for patients. The Nurse Case Manager is responsible for helping patients and families to identify all precertification needs for MAT prescribed. The Nurse Case Manager provides, and coordinates all required inpatient treatment patient education. Additionally, the Nurse Case Manager is responsible to ensure all patients medical aftercare appointments and scheduled.

Specific Responsibilities:

Actively participate in the development and continued assessment of individual treatment plans.

Act as an advocate for all patients when working with external medical providers and coordinate medical care on their behalf.

Create and manage the individual treatment of care for patients with chronic or serious conditions such as diabetes, heart disease, COPD, and other chronic concerns.

Advocate for personalized treatment options that address a patient's unique care needs.

Schedule patients' medical appointments, communicate about a patient's health condition with the patient and their family.

Offer education and guidance for navigating complex medical decisions and support communication between external providers.

Serve as a liaison between patients and their insurance providers to coordinate quality, cost-effective care with the best patient outcomes.

Work collectively with the clinical team to engage, educate, and coordinate patient care with the patient, their identified supports, and community providers to ensure all services prescribed are administered prior to treatment completion.

Implement case management services in accordance with the individualized treatment plan beginning with obtaining consent for points of contact and coordinating care with all collateral contacts for whom there is consent.

Directly Provide Medicated Assisted Treatment, Smoking Cessation, Infection Control and Infectious Diseases education Collaborate and communicate with the patient, family, significant others, identified support systems, community providers, referral sources, medical providers and care coordinators about the clinical treatment plan including available options for ongoing treatment.

Articulate a thorough explanation of the advantages and disadvantages of all available resources and services for ongoing treatment programs to patient, referent, families, and other stakeholders in patient treatment.

Document regularly in the medical record ongoing progress towards case management treatment goals.

Education and Experience:

Current/Active State Nursing Licensure. (RN preferred)

Minimum of one (1) year experience in Behavioral Health, Substance Abuse Treatment, or Psychiatric setting Knowledge of healthcare, detoxification process, addiction and co-occurring disorders, DSM and ASAM criteria, and terminology surrounding all of the above.

Ability to read and interpret written information; write clearly and informatively; and edit work for spelling and grammar.

Ability to speak clearly and persuasively in both positive and negative situations; listen and obtain clarification when necessary; respond well to questions; demonstrate group presentation skills and actively participate in team meetings.

Working knowledge of Microsoft Programs (Word, Excel, and Outlook).

Decision Making: use effective approaches for choosing a course of action, developing appropriate solutions, and/or reaching conclusions; implement action plans consistent with available facts, constraints, resources, and anticipated consequences; demonstrate confidence in the work done to manage challenging situations

Collaboration: must be able to work in collaboration with other professionals and leaders across several disciplines, ability to motivate treatment team towards discharge planning when appropriate and obtain recommendations for ongoing treatment

Work Environment: May work in a variety of environments including professional offices, clinics, hospitals, or out-patient facilities. They spend much of their time on their feet, actively working with patients.

Physical Demands: While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision and the ability to adjust focus.

Travel: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
