Mountain View Hospital is looking for a Discharge Planner/Nurse/Social Worker to join our team!
JOB SUMMARY:
Assists patients and their families with personal and/or environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Prepare patient and family for discharge from acute care hospital setting. Works in close collaboration with physician health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient’s health problems.

DUTIES AND RESPONSIBILITIES:
1. Coordinate the discharge plan and services with the patient, their families and other disciplinary teams to ensure patient has an appropriate plan and services when ready to discharge. Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations.
2. Reviews and validates physician orders ensuring they are carried out, reports progress and unusual occurrences on patients to department supervisor.
3. Ensures cost effective and appropriate health care services are provided to all patients regardless of patients funding. Assists the department head in the supervision of the utilization of resources to promote quality, cost-effective outcomes for the patient and the patient’s family.
4. Collaborates with other departments in discharge planning process, providing clear communication and feedback.
5. Assists in assessing the physical, functional, social, psychological, environmental and financial needs of patients.
6. Assists with referrals to appropriate community resources and contributes to the assessment of patient’s support systems.7. Acts as a patient advocate by assisting in acquiring appropriate medical, nursing or other health care in the home or clinic setting as needed upon discharge.
8. Maintains accurate patient records, ensuring patient confidentiality.
9. Contributes and assists in the performance of case management activities, and contributes to the assessing, planning, implementing, coordinating, monitoring and evaluating of the needs of patients for services both during and after hospitalization.
10. Provides service planned to restore patient to optimum social and health adjustment within patient's capacity.
11. Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability.
12. Participates in planning for improving health services by interpreting social factors pertinent to development of program.
13. Assist with patient emotional support of patient and family members as needed.

QualificationsEducation/Certification: Could be a graduate from an accredited school of nursing with current license as an LPN or RN in the state of Idaho, with preferred experience in Case Management. Or, a Licensed Social Worker, Bachelor's degree (B.A.) in social service from four-year college or university; preferred experience in a healthcare facility.
Experience: 3+ years Clinical Nursing or Relevant Social Work experience
Equipment/Technology: Basic computer knowledge.
Language/Communication: Effective written and oral skills. Ability to comprehend and follow detailed instructions. Ability to effectively present information and respond to questions from groups of managers, patients, families, and the pressure during difficult circumstances. Speaks clearly and persuasively in positive or negative situations: listens and gets clarification
Mathematical Skills: Business related math.
Mental Capabilities: Ability to organize, recognize, and correct Inefficiencies, breakdown in system or communication and be able to assist with problem solving on the fly. Handle and prioritize multiple requests.
Interpersonal: Can remain pleasant under extreme pressure and during difficult circumstances. Ability to be cheerful, trustworthy, loyal and organized, with the ability to demonstrate professional behavior at all times, as an active member of the care team.
