Why does the job exist?
Incumbent will be responsible for ensuring compliance with the CARA state law that became effective July 1, 2019. This law (CARA) requires the creation of a Plan of Care for every newborn with in-utero substance exposure and provide an opportunity to offer supportive services for parents with substance use and their infants. The CARA navigator will be responsible for completing comprehensive family assessments,building working relationships with families and support them in accessing needed services.
How does it get done?
- Review every plan of care to make sure the plan is complete. Assure insurance Information is captured on the mom/newborn to allow for correct referrals to the right Insurance for Care Coordination. 10%;
- Complete comprehensive family assessments with families who have a CARA plan of care. Revise plan of care as necessary based on needs identified in family assessments. 20%;
- Conduct home visits with families who have a CARA plan of care to support engagement in services. Coordinate with the individuals involved in the Plan of Care to ensure engagement and compliance with state law and rules regarding CARA. Contact Statewide Central Intake (SCl) when there are safety concerns. 20%;
- Transport families as needed to identified support services 10%;
- Assist in training providers and individuals on the CARA law and how to create a plan of care 5%;
- Provide outreach to target populations throughout the state 10%;
- Provide technical assistance to providers as needed on CARA and plan of care 10%;
- Manage and participate in the ongoing CARA work-group by developing agendas and leading work-group discussion in topics related to CARA 10%;
- Work with other state agencies in continued implementation of CARA across various sectors 5%.
Minimum Qualification
Bachelor's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology and/or Family Studies/Services and four (4) years of any combination of experience including working with communities, working on health or social service related matters, social work/case management experience, behavioral health and/or health care. Employment Requirements
Must possess and maintain a valid Driver's License. Pre-employment background investigation is required and is conditional pending results.
Working Conditions
Work is performed in an office setting: late hours and weekends work may be required. Will be exposed to regular periods of video display terminal and keyboard usage and stressful situations. Travel may be required. Incumbent will work under stress and frequent time constraints.
