A Member of the interdisciplinary team within the Behavioral Medicine, Social Work team. Responsible for patient advocacy, referrals, and discharge planning for patients and families. Makes reports, communicates with external agencies as required, participates in the discharge planning process, aids in implementing treatment goals, following policies and procedures.
EDUCATION AND EXPERIENCE:
Associate Degree in Social Work, Psychology, Sociology, or related Human Services field required. One year of acute care or social agency experience in order to gain necessary skills and knowledge of discharge planning and community resources.
WORKING CONDITIONS: Normal patient care environment with little exposure to noise, dust, temperature and the like. Occasional (up to 20% of work time) exposure to communicable diseases and infections when working with patients. Occasional (up to 20% of work time) exposure to physical injury working with combative patients. Generally (up to 50% of time) ability to remain stationary or move about unit when working with patients. Ability to travel to various locations, exposure to weather when traveling outside.
