40 hours every two weeks (.5FTE), benefit eligible position
Every third weekend scheduled 0800-1630, remainder of shifts flex with hours between 0700-0300.
This position is split between the ED and inpatient units.

Required: Michigan Licensed Master Social Worker (LMSW)
The Medical Social Worker is a qualified professional who provides medical and social services to patients in the emergency room and inpatient hospital setting with the physician's orders and under the supervision of Behavioral Health Services. This position requires the ability to understand, assess, treat, care for, and react effectively to the patient population receiving services (adolescent, adult and/or geriatric). Reports to the Behavioral Health Emergency Services Manager.

Required: Michigan Licensed Master Social Worker (LMSW)
PERFORMS COMPREHENSIVE PSYCHOSOCIAL PATIENT ASSESSMENT:
- Completes initial patient contact in a timely fashion.
- Accurately determines patient condition and medical/social needs based on interview and assessment of surroundings.
- Collaborates with fellow team members in understanding the social and emotional factors related to patient health problems.
FORMULATES APPROPRIATE PATIENT TREATMENT AND DISCHARGE PLAN:
- Develops patient plan of care in collaboration with the physician and fellow care team members.
- Customizes plan to the individual patient, based on clinical assessment of psychosocial, medical and financial factors affecting the patient.
- Determines visit frequency appropriate for individual patient need.
- Conducts ongoing visits and case conferencing in a timely fashion when applicable.
- Provides consultation and coordination of services with other disciplines as needed.
- Connects client/family with appropriate community resources as needed.
EVALUATES TREATMENT PLAN EFFECTIVENESS DURING CARE:
- Completes daily logs, client record documentation and clinical and progress notes in a timely and accurate fashion when applicable.
- Collects, evaluates and utilizes appropriate data for problem solving.
- Monitors patient progress, seeking and evaluating feedback from patient, significant other, family and health care team members.
- Collaborates with patient, significant other, family and health care team members to modify plan appropriately as needed.
- Communicates concerns to other disciplines as needed.
OTHER:
- Participates in in-service programs when required
- Masters Degree in Social Work from school approved by Council of Social Work Education required
- Minimum one year of work experience in a healthcare setting (hospital, clinic, rehab center, etc) preferred
-Current Michigan LMSW license required
- Current Michigan LCSW license preferred
- BLS for the Healthcare provider by 60 days after hire required
- Current Valid Michigan Drivers License without restrictions and no more than one violation in the last three years required
- Knowledge of available community resources with effectiveness in referring to such resources required
- Knowledge of reimbursement eligibility and processes required
- Knowledge of Medicare, Medicaid and other payers required
- Strong therapy / counseling skills required
- Strong implementation / interventional skills required
- Strong organizational, prioritizing and time management skills required
- Must be flexible / adaptable with assignments and scheduling
