Job Summary:
Responsible for the direct Social Work case management services to the participants of the Thome PACE program. The Social Woker works under the direct and indirect supervision of the Social Work Supervisor or designee but is independent in the application of advanced Social Work knowledge and skills. Provides such social services support as participant screening, case management, counseling and referral. The Social Worker is a member of the Interdisciplinary Team (IDT) and participates in the assessment and review process of participants in determining eligibility and care needs. The Social Worker provides care in a variety of settings including, but not limited to the day center, the participant’s home, and the hospital.
Skills and Knowledge
Must be able to read, write, speak and understand the English language and follow written and oral instructions.
Must display patience, tact, a positive disposition, and enthusiasm, as well as be willing to work with a variety of participants with varying cognitive levels.
Knowledge of the principles of human behavior, individual and group therapy techniques, short-term interventions, substance abuse assessment and treatment, and relevant regulations.
Individual therapy skills for geriatric participants, analytical thinking and problem-solving abilities.

Required Qualifications
- Must possess a master’s degree (MSW) from an accredited School of Social Work.
- Current License as a Social Worker in the State of Michigan. (LLMSW, LMSW)
- Must be CPR certified or willing to obtain CPR within 90 days if offered employment.
- 1 year experience with elderly (preferred)
