Working within the framework of Vista Prairie Communities’ values of Caring, Respect, Collaboration, Innovation, and Stewardship, the role of the Director of Health Services is to ensure that the health and safety of all residents are met and to ensure that the facility complies with current regulations and standards.
KEY ACCOUNTABILITIES
- Deliver and manage care through the Aspire Lifestyle Model to honor residents and enhance their quality of life.
- Engage residents using the Aspire 5 pillars of wellness including Social, Physical, Intellectual, Emotional, and Spiritual.
Assessments
- Completes initial, face-to-face comprehensive assessment of prospective/new residents and develops a proposed service plan.
- Conducts or delegates the completion of assessments as resident conditions change and as required by regulation.
Case Management
- Monitors and maintains resident’s clinical charts and progress notes.
- Communicates with physicians and other personnel regarding changes in resident’s needs or conditions.
- The RN controls management services identified in the individualized service plans.
- Assists with the start-up of services, transitions, and discharges.
- Updates and communicates changes to resident records promptly each workday.
- Ensures all physicians’ orders are signed and implemented on a timely basis consistent with the resident's needs and condition.
- Addresses, documents, and reports resident and family concerns or complaints.
- Ensures that documentation of resident services and orders are properly filed and conducts periodic chart audits.
Compliance with HC Licensing Requirements
- Ensures that proper systems, policies, and procedures are developed and implemented correctly by licensed and unlicensed staff regarding the storage, set up, administration, and disposal of medications, including systems to control controlled medications.
- Is responsible for implementing the facilities’ infection control plan, universal precautions, and TB prevention plan, and for implementing and documenting TB screening of staff and other vaccinations.
Resident Service Planning and Delegation of Tasks
- Develops, reviews, and implements service agreements with all new Reviews and revises service plans as needed due to changes in resident’s needs and at a minimum will review the plan once every 12 months with the resident and/or the resident's representative.
- Provides all new unlicensed staff with required training according to state law for all delegated nursing tasks.
- Determines the staff’s competency to provide all delegated tasks according to the procedures for the resident.
- Develops and assures that all direct care staff has at least 8 hours of in-service training on required topics for every 12 months of work and assures that staff clearly understands and follows all resident care procedures.
- Assures that each staff person is oriented to the individual needs of each resident; communicates changes to plans of care.
- Supervises medication practices and medical equipment being used in the building.
- Provides direct supervision of new licensed and unlicensed staff within 30 days of hire, and on an on-going basis.
- Assists in the development of policies and procedures regarding the storage, set up, administration, and disposal of medications, including systems to secure controlled medications.
- Ensures that residents' medication records are up to date and administration procedures are followed properly; investigates and discusses errors and issues with staff and implements corrective action plans, as needed.
- The position may be on-call 24-7.
- Evaluates staff performance through appropriate documentation.
- Re-train or discipline staff as needed for non-compliance with job performance or agency policies and procedures.
- Develops and maintains a system to track the training and competencies of all unlicensed staff and provides continuing education and documents such training to ensure compliance with state, federal, and local regulations.

EDUCATION AND EXPERIENCE
- Must be registered and licensed with the State of Minnesota/Iowa/Wisconsin as an RN and must maintain a current unencumbered license with the Board of Nursing.
SPECIFIC REQUIREMENTS
- Must be a minimum of twenty-one (21) years of age.
- Ensures compliance with regulations and is available for federal and state licensure surveys.
- Must be able to relate information concerning residents’ condition.
- Must be knowledgeable about validation techniques, reality orientation, and/or appropriate diversional activities for clients with dementia.
- Must possess the ability to make independent decisions when circumstances warrant action.
- Must receive a "not disqualified" criminal background result and may not be on the OIG exclusions list.
- Stays up to date on all required training.
- Understands and adheres to the Vulnerable Adult and Abuse Is familiar with the Resident’s Bill of Rights and adheres to it.
- Working knowledge of state, federal, and local regulations.
