Lovelace Health System (LHS) has been committed to meeting the growing healthcare needs of the Albuquerque community since 1912. POSITION SUMMARY
Directs and administers strategic, operational, fiscal and personnel activities of the Case Management department to ensure procedures adhere to standards of various accrediting and regulatory agencies. Prepares and manages budgets, manages/supervises staff and ensures quality services are provided through daily operations. Manages and oversees plan of care for patients. Serves as a liaison and provides support. Adheres to policies, procedures and regulations to ensure compliance and patient safety.
Responsibilities
- Directs and administers strategic, operational, fiscal and personnel activities of the Case Management department to ensure procedures adhere to standards of various accrediting and regulatory agencies.
- Manages and oversees plan of care for patients. Interfaces with family members to ensure understanding.
- Represents the organization and/or department at various community and/or business meetings; promotes existing or new services; serves as liaison and develops and maintains business relationships.
- Takes ownership of assignments, special projects, pilot programs; other duties as assigned or requested.
- Communicates and listens effectively with internal and external customers; effectively understands instructions and shares knowledge.
- Cooperates and interacts with supervisors, peers, other departments and all customer groups demonstrating our commitment to “service”.
- Meets budget (Operating & Capital) for assigned areas.
- Mentors, coaches, develop subordinates to accomplish goals. Educates others regarding the role and services associated with Case Management.
- Develops policies, processes, strategies and goals that support Department/Corporation direction.
Qualifications
Education & Experience
- Minimum: Bachelor’s Degree in Nursing, business or related field of study; Desired: Master’s Degree
- Five (5) years of Case Management experience of which three (3) years must have been in a management capacity.
- Relevant education and/or experience may be substituted for one another in accordance to policy.
- Valid NM Nursing license or NM compact license (within 90 days of hire) or current Case Management Certificate/Fellowship Status; employee will be required to maintain license and requisite CEU’s.
