The Direct Support Professional is responsible for implementing person-centered services for each individual. This includes ensuring implementation of the individual’s goals/valued outcomes in relation to both center-based and community inclusion activities. In addition, the Direct Support Professional is directly responsible for the individual’s overall safety and well-being. Requirements:
- GED/HSD Required
- AMAP, CPR & SCIP preferred
- Valid NYS Driver’s License Required
- Flexible - can work various shifts (days, evenings, nights, weekdays/weekends), strongly preferred.
- One or more years’ experience working with individuals with developmental disabilities preferred.
- OPWDD experience a plus
Responsibilities:
- Transport individuals in agency vehicles, as required for community inclusion, recreational purposes and doctor’s appointment.
- Will assist individuals in any and all activities which are designed to foster independence, personal community growth and social maturity.
- Support and teach individuals according to their services, goals, and outcomes. Actively engage individuals in the skills development, socialization and task participation across all settings.
- Participate in planning group recreational trips and outings.
- Assist in timely collection of consumer-related information, including behavioral data, goal-related data, community inclusion logs, communication books, etc., ensuring their accuracy and completeness.
- Provide crisis intervention and counseling as required.
- Report all incidents, including allegations of abuse and neglect and serious reportable incidents, to program management immediately upon discovery.
- Advocate appropriately and professionally on behalf of program participants, as required
