Job description

Central Ohio Primary Care Physicians is seeking a Diabetes Care & Education Specialist for our Columbus Endocrinology office located in Columbus, OH.  Registered Nurse (RN) or Registered Dietician Nutritionist (RDN) are encourage to apply! This position provides diabetes self-management training and support to COPC clients. Focused on meeting individuals where they are and helping them learn about how to stay healthy with diabetes. We evaluate medication therapy, BG levels and report feedback to the referring provider to help support optimal care. We provide evidence-based guidelines on cardiometabolic health related to diabetes wellness. This will be a full-time role working first shift hours Monday-Friday.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Complete a comprehensive assessment for each person including emotional and behavioral health, interpret personal health data, assist in development of a care plan based on person’s assessed needs and goals and promote safely managed BG, B/P and lipid levels.
  • Facilitate discussion in both group and individual sessions to help identify barriers, strategies, current guidelines, etc. to help support well-managed diabetes and pre-diabetes.
  • Communicate effectively with the health care team related to findings and needs to help support optimal clinical outcomes. Document in the employee health record (EHR).
  • Participate with the patient's healthcare team to promote optimal outcomes.
  • Provide training and support on the use of medical devices that assist in the management of safe BG levels. This includes but is not limited to syringe/vial method, pen devices/apps, insulin delivery devices and cgm.

Education, Licensures & Certifications

  • Required: Bachelor’s degree RN or RDN
  • Required: Licensed in the state of Ohio
  • Preferred: Certified Diabetes Care and Education specialist or ability to sit for exam within 24 months of starting position.
  • Preferred:  3 years of previous experience in diabetes education.
