Job Description
The Mary Ellen Falzone Diabetes Center is our region's first facility dedicated to Diabetes Education and care. The center is a state-of-the-art facility located on the north campus of ProMedica Toledo Hospital. Our expert, multidisciplinary team of endocrinologists, diabetes educators, dietitians, midlevel providers, and pharmacists help our patients with diabetes counseling, diabetes support, and medication therapy management.
Position Summary:
The Dietitian, Clinical Educatorcollaborates with the multi-disciplinary team to provide Diabetes Education to pediatric, adult and geriatric patients and families as well as provide Medical Nutrition therapy, when appropriate, in ambulatory program settings. This includes assessment, creating care plans and implementing the education programs as related to diabetes, diabetes in pregnancy and other nutritional disorders. Other duties include, but are not limited to:
- Serves as a member of the multidisciplinary team as it relates to Diabetes Self- Management Training (DSMT)
- Serves as a resource to dietary department dietitians for diabetes and diabetes in pregnancy DSMT and Medical Nutrition Therapy (MNT) as needed
- Performs outpatient DSMT and/or MNT for pediatric, adult and geriatric patients as ordered by physician, both individually and in group settings, both confidently and effectively with role model expertise
- Provides a variety of education materials and approaches to individualize DSMT and/or MNT to accommodate any patient barriers to learning
Job Requirements
- Must have a Bachelors in Dietetics
- Must be a Registered and Licensed Dietitian in the State of Ohio
- Must be able to communicate and respond to inquiries; requires effective interpersonal skills
- Must be able to input and retrieve information from computer. Complete training and certifications required to provide education on current treatment options and technologies; including, but not limited to insulin infusion pumps, continuous glucose monitors and professional blood glucose monitors.
- Knowledge of diabetes, diabetes in pregnancy, treatment, teaching techniques and adult learning
- At least 1 year of experience in Diabetes Education
