• The employee must be able to work from the Grants Pass, Oregon office at least 2 days per week.
• The employee must be able to work from a home office up to 3 days per week (if they elect the work from home hybrid option).

Summary

This position is responsible for supporting in the design, development, implementation, and coordination of complex data compilation and analysis that promotes and supports population health management initiatives by performing the following duties.

Essential Duties

• Creates and maintains detailed reporting to identify cost drivers and negative trends within medical and/or pharmacy data.
• Articulates understandable explanations of complex clinical data or trends to all staff through oral, written, and graphical presentations.
• Designs utilization management, cases and disease management dashboards and/or models and reports to support Population Health department clinical staff.
• Integrates data from multiple reports to evaluate population health and triages high risk patients for disease management and case coordination.

Education & Experience

• Bachelor's degree (B. A.) from four-year a college or university; or two to four years related experience and/or training; or equivalent combination of education and experience.
• Three years’ experience in the healthcare industry is required.
