We tailor make care plans that works for our clients whether transitioning from the hospital to home or companionship for a few hours a day.

The Community Resource Specialist is a member of the Care Advantage under the direction and supervision of the Regional Vice President of Operations. The CRS performs various professional duties to include the coordination of community events and acting as a liaison between the community and Care Advantage by assisting community members with service coordination. In addition, the Community Resource Specialist is responsible for being a resource to the community, as well as organizing events and coordinating distribution of leads that result from events.

• Develop new referral sources by planning and organizing schedule to call on potential referral outlets and other sources of prospective clients.
• Meets with prospective clients in their homes to discuss agency's services, payment options, onboarding process.
• Assists the agency's Client Relationship Managers in processing and on-boarding new clients.
• Communicates with and supports existing clients as needed in conjunction with PHC's Client Relationship Managers.
• Keeps management informed by updating CRM systems, submitting sales activity and results reports, such as daily call reports, weekly work plans, and monthly and annual territory analyses.
• Develops "out of the box" sales strategies/models and evaluates their effectiveness.
• Assist with developing recruitment strategies to attract candidates.
• Collaborates closely with Recruiting and Human Resources to ensure candidates complete onboarding process.

Requirements:

• Associates degree or higher in Social Work, or an associated field; or equivalent experience preferred.
• Bi-lingual (English and Korean) - Required.
• Minimum of two years' experience working with direct client/patient support and communications with a proven track record of customer service excellence.
• Experience working with the Medicaid application process.
• Comfortable working collaboratively in cross-functional team or independently.
• Excellent verbal and written communication skills.
• Possess proficient computer skills with knowledge of Microsoft products.
