: OnPoint is seeking an energetic, organized, committed professional to fill the position of full-time Community Nurse. As a Community Nurse, you will provide nursing services to OnPoint consumers who have a mental health, substance use or co-occurring disorder or intellectual/developmental disability, primarily in community-based settings; this includes private residences, adult foster care, and other non-medical residential placements. Target population includes adults and children who require nursing support in the community to effectively manage behavioral health symptoms, often while experiencing physical health challenges as well. Includes health screenings and appraisals, nursing assessments, medication reconciliation, injections, and health promotion, along with care coordination for chronic health care conditions. Deliver services through a team-based approach with OnPoint case managers, clinicians, peer support specialists, and peer recovery coaches, and primary care and other providers as necessary comprehensive and coordinated care. The goal is to support consumers in full community participation in employment, education, social and recreational activities.
PAY RANGE/BENEFIT PACKAGE:
Salary: starting at $64,874 annually - placement above minimum salary is based on experience.

- Registered Nurse degree with a State of Michigan License required.
- At least three years of nursing experience with people diagnosed with mental health, substance use or co-occurring disorders or intellectual/developmental disabilities.
- Must be a strong advocate of self-determination and a person first philosophy.
- Must be able to be empathetic to the target population and their families and caregivers.
- General working knowledge of the issues faced by the target population.
- Valid Michigan’s driver’s license, operating vehicle that is insured in accordance with agency standards, and a good driving record.
- Knowledge of psychiatric/behavioral health medications preferred.
- Experience in a public behavioral health setting preferred.
- Community-based experience serving the target population preferred.
- Perform medication injections, RN medication reviews, health screenings, nursing assessments, health appraisals, and assist with community partners’ nursing needs.
- Provide care coordination for chronic health care conditions.
- Provide behavioral health screening and assessment, intervention, and case management consistent with funding, regulatory, agency, and best practice standards.
- Participate in various non-direct service activities including supervision, interdisciplinary team meetings, staff meetings, treatment planning, quality improvement, utilization review, and continuing education.
- Performs other duties as assigned.
