The Nurse is responsible for Integrated Health Care Services for persons served at Lapeer County Community Mental Health. This position requires extensive collaboration, education and training with the case holders, persons served, and the medical community. This position shall also provide comprehensive nursing services, including screenings and assessments, clinical services, consultation, teaching, and training to persons with mental health disabilities.
Essential Functions
1. Provides health screenings and nursing assessments.
2. Monitors and evaluates health-related tests and reports for persons served.
3. Develops and implements service plans related to nursing and health care needs
of persons served, amending and updating as indicated.
4. Oversees use and maintenance of medications, including medication
administration-related activities and agency stock medications.
5. Provides clinical nursing services as required.
6. Provides health care related training and consultation to non medical staff as
required.
7. Provides ongoing case management and supports coordination services as required.
8. Participates in crisis intervention and referral services as required.
9. Develops and maintains effective working relationships with other community
agencies.
10. Completes clinical documentation in a timely manner, consistent with agency,
state, and federal requirements.
11. Works closely with Integrated Health Liaison to implement integrated health care
services for persons served.
12. Works closely with case holders to implement health related goals.
13. Conducts health related groups for persons served at LCCMH and off-site as
specified by the supervisor, director or their designee.
14. Coordinates health care services with other health care providers in the
community.
15. Participates as a member of the Integrated Health Care Team.
Other Functions
1. Compiles and prepares reports as assigned.
2. Provides orientation to new employees regarding health safety and infection
control policies and procedures.
3. Provides periodic in-service training to staff related to health safety and infection
control issues.
4. Participates in training and continuing education opportunities as assigned.
5. Works on achieving agency wide goals related to the Integrated Health Care
Activities.
6. Provides Quarterly updates to the Integrated Health Care Work Plan.
7. Participates in state-wide Integrated Health Care initiatives and training.
8. Provides leadership related to Integrated Care initiatives and champions
Integrated Care treatment strategies.
9. Serves on agency committees as assigned by the Mental Health Supervisor.
10. Other duties as assigned by the Mental Health Supervisor.
Education: Graduation from an accredited school of nursing. Bachelor’s degree in nursing preferred.
Experience:
One to two years of nursing experience, preferably working with persons with mental health-related disabilities.
Lived experience with behavioral health issues preferred.
Other Requirements:
1. Current State of Michigan licensure as a Registered Nurse.
2. Ability to write and speak effectively.
3. Possession of a valid Michigan’s driver’s license.
4. Ability to establish and maintain productive working relationships with persons
served, staff and the medical community.
5. Ability to relate to the public and staff with tact, courtesy and good judgment.
Salary: Entry: $ 60,396.54 6 months: $64,939.26 1st Year: $69,481.98 2nd Year: $74,911.62 3rd Year: $80,341.25 4th Year: $86,160.26
