Community Health Worker - Family And Community Medicine

The bilingual (Spanish and English) Community Health Worker (CHW) will assist individuals and communities to adopt healthy behaviors at the Wyss Wellness Center in South Philadelphia. The CHW will conduct outreach to Wyss patients with diabetes and help patients and their families navigate and access primary care and Community resources and services. The CHW will provide support to the selected Wyss center providers and social worker through a team based care model. The CHW may also assist with delivering wellness classes focused on diabetes and heart disease.
ESSENTIAL FUNCTIONS:

• Assists in developing and implementing care plans in cooperation with patients, families, and care providers
• Identifies and help enroll eligible individuals in federal, state, and local private or nonprofit health and human services programs
• Serves as a liaison between patients/families/communities and healthcare agencies
• Provides Guidance and social assistance to patients. families and Community residents
• Enhances patients' ability to effectively communicate with healthcare providers
• Provides culturally and linguistically appropriate health education materials within the scope of training
• Provides referral, follow-up services and schedule patient appointments as needed
• Assists patients and families in understanding and carrying out their healthcare providers' advice
• Follows-up whether patients are receiving and adjusting well to their care plans, providing additional resources as necessary.
• Compiles and tracks community resources available to and used by Jefferson's patient population
• Interacts with co-workers, visitors, and other staff consistent with the iSCORE values of Jefferson.

Qualifications
EDUCATIONAL/TRAINING REQUIREMENTS:
High School Diploma or GED
CERTIFICATES, LICENSES, AND REGISTRATION :
Certified Community Health Worker training program preferred.
EXPERIENCE REQUIREMENTS :
Prior experience in community outreach preferred.
Conditions of Employment
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
Conditions of Employment
Covid Vaccination is a requirement for employment at Jefferson for employees working at Jefferson's clinical entities or at the University.
