Job description

Choice Consulting is a Collective, Holistic, Outreach, Inclusive, Community Empowerment organization designed to address Social Determinants of Health with evidence-based approaches to service delivery.

The Community Health Worker (CHW)/Enrollment Assister position will conduct tasks with regards to providing fair, impartial, and accurate enrollment assistance and information to individuals, and families about the health insurance options available through the state of Pennsylvania Health Insurance Marketplace Pennie including Qualified Health Plans, free to low-cost coverage from Medical Assistance (MA) Medicaid and the Children’s Health Insurance Program (CHIP) Medical Assistance for Workers with Disabilities (MAWD).

The Community Health Worker (CHW)/Enrollment Assister will conduct outreach education and enrollment assistance in a manner that is culturally and linguistically appropriate to the needs of the community being served.

Responsibilities

This is a fully remote position requiring extensive travel within the assigned communities.

The Community Health Worker (CHW)/Enrollment Assister will conduct all related job functions assigned to the Community Health Worker (CHW)/Enrollment Assister, including but not limited to:

  • Educate and assist individuals who self-identify as uninsured or underinsured with understanding the various health insurance coverage options that may be available to them.
  • Assess enrollment eligibility and assist in the completion of initial/renewal applications for Medicaid (Medical Assistance), Children’s Health Insurance Program (CHIP) and Pennie.
  • Explain to members the benefits of enrolling children in free to low-cost insurance available through the Commonwealth of Pennsylvania.
  • Cultivate relationships and engage with community-based organizations (CBOs), Federally Qualified Health Centers (FQHCs), and regional provider systems and provider sites.
  • Facilitate connections (i.e., outreach) and referrals (i.e., encounters) between individuals and community-based agencies to address Social Determinants of Health (SDOH) needs.
  • Identify and communicate local and regional community-based needs related to SDOH.
  • Provide courteous, professional, and confidential assistance to all consumers seeking healthcare access and information.
  • Serve as a resource for consumers, community-based organizations and small business owners seeking to understand the Marketplace, health insurance and healthcare options.
  • Conduct marketing and outreach within local communities about health insurance options through the health insurance marketplace.
  • Work independently, travelling throughout communities and providing enrollment services in various locations.
  • Comply with policy on confidential information in addition to any applicable laws governing the confidentiality of enrollee financial information, social security numbers, personally identifiable information, and health information, including Medicaid data.
  • Ensure that follow-up and data entry is completed to capture consumer referrals and outcomes.
  • Achieve performance target, working closely with the other CHW’s, Enrollment Assisters and community partners to identify and overcome challenges.
  • Comply with all continuing education and recertification requirements.
  • Other duties as assigned.

Education/Experience

  • Bilingual - English/Spanish is required.
  • Bachelor’s Degree preferred or a minimum of 2-4 years of Work-Related Experience
  • Previous Experience in the Healthcare Industry
  • Experience with Health Care Policy and Ethics Compliance and Patients’ Rights
  • Experience with Medicaid
  • Experience with Physical and Behavioral Health Medicaid/Managed Care
  • Effective and strong communication skills.
  • Strong interpersonal skills: must be a team player who works cooperatively and effectively with other Choice Consulting staff and community-based partners.
  • Respects differences of race, ethnicity, age, gender, sexual orientation, gender identity, religion, ability, and socio-economic circumstance; understands and applies principles of cultural competence.
  • Must be able to handle multiple assignments and take initiative to plan and complete projects effectively.
  • Strong presentation and organizational skills

  • Must be able to work flexible hours, including evenings and weekends.
  • Must have knowledge of and maintain the standards and requirements of the Health Insurance Portability and Accountability Act (HIPAA). Ensure the protection and security of personal, confidential, and identifiable information in a professional and responsible manner and carry out all measures to prevent unauthorized disclosures.
  • Proficient in MS Office (Word, Excel, and Outlook) within a Windows-based environment, Internet applications, and electronic documentation programs.
  • Other duties as assigned.

License/Certification: Community Health Worker

  • Driver’s License
  • Current Valid Car Insurance
  • Successfully acquire Enrollment Assister Certification (after start date)
  • Successfully acquire Community Health Worker Certification (after start date)
