Community Health Worker

Squirrel Hill Health Center (SHHC), a dynamic and growing Federally Qualified Health Center, is hiring a full time Community Health Worker. The Community Health Worker (CHW) is a grant funded front-line staff member who is also a member of the community that they serve. This relationship allows the CHW to serve as a bridge between SHHC and the community to facilitate access to health services and break down cultural barriers to care. The CHW will address the health needs of the community through community education, patient outreach and clinic visit support.

Squirrel Hill Health Center strives to create a welcoming environment, with special sensitivity for patients’ race, national origin, primary language, religion, age, sexual orientation, gender identity and disability status.  SHHC has a diverse patient base, with half of all patients speaking a primary language other than English.  The specific community focus of this CHW position would be driven by the applicant’s background, community ties, experience and interests. 

RESPONSIBILITIES

• Build and maintain relationships with community members and connect them with care as well as SHHC patients and support them as they work toward healthcare and wellness goals.
• Deliver Community Education
• Outreach SHHC patients to close gaps of care.
• Provide clinic visit support by attending select SHHC medical appointments to assist in identifying needs and barriers to their care plan.
• Be present on the SHHC mobile unit when it is located in neighborhoods aligned with CHW focus community.
• Assist with language translation if appropriate.
• Collaborate with SHHC teams.

QUALIFICATIONS

• Current COVID 19 vaccination, flu, Tdap, proof of MMR and Hep B vaccines required
• High school diploma, GED, or higher
• Complete and maintain CHW certification
• Commitment to connecting people with resources
• Knowledge of area social service networks, health insurance issues
• Interest in immigrant and refugee populations
• Valid driver's license and access to automobile
• Knowledge of Microsoft suite
• Second language a plus
