The CHW will serve as a liaison/link/intermediary between health system and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery. Works to improve health outcomes and eliminate racial, ethnic, and economic disparities within the populations through outreach, social support, and advocacy. The CHW supports the attributed patient population with resources related to social influencer of health leading to better patient outcomes. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.
