Position Overview

The Dental Community Health Worker (D-CHW) must abide by various regulatory compliance rules and must always treat patients with dignity and respect.  The aim of their work is to increase access to oral health care for children ages 6 to 11. The CHW will work directly with the patients to help them navigate and access dental services while adopting healthy oral behaviors.

General Responsibilities

• Implement care coordination strategies, with specific target population, that promotes, maintains, and improves oral health care for patients.
• Maintain internal and external referral lists for follow-up visits
• Collecting Patient Interaction Tracking data for the MNOHI project to trace all patient contact
• Assigns and complete annual Smiles for Life training
• Liaise with the Pediatrics clinic to bridge oral health care and education gaps
• Communicate with patients via Care Messaging system
• Communicate with school nurses at School-Based care sites.
• Other duties as assigned

Qualifications

• High School diploma or equivalent required
• EPIC experience preferred
• Dentrix experience preferred
• 2-3 years of health/social services experience preferred
• Windows-based computer experience required

Pay: From $17.00 per hour
