NOW HIRING: Community Health Worker
The Community Health Worker (CHW) will work closely with individuals in the community who have chronic health conditions, substance use/behavior health diagnosis, women of childbearing years, and/or who experience obstacles in obtaining healthcare services. The CHW will assist clients in finding community resources which reduce their barriers to care and help ensure that the client is knowledgeable about their health condition(s). This position focuses on providing excellent customer service to internal and external customers in accordance with the mission, core values, and purposes of Franklin County Public Health.
Duties Include:
- Client outreach and enrollment activities by conducting neighborhood outreach to vulnerable, underserved populations.
- Linking eligible participants of the Central Ohio Pathways HUB to FCPH programs and culturally tailored interventions.
- Identifying barriers to care and services in areas such as education, transportation, housing, health insurance, COVID-19 vaccination and understanding procedures and language barriers.
- Educate clients about the importance of preventative health care; provide basic education to ensure an understanding of their health conditions.
- Attend local events and disseminate health related information within the community.
- Utilizes appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds, sexual orientations, lifestyles and physical abilities.
- Performs public health emergency response duties as assigned and consistent with job classification and training provided, in response to threats to the public’s health. Participates in Department and community emergency response training and drills in support of disaster preparedness consistent with job classification.

Requirements:
- High school diploma or GED is required. Bachelor’s degree in public health, public administration, social work or other human services related field preferred.
- Community Health Worker (CHW) Certificate through the Ohio Department of Nursing is required.
- Must possess a valid driver’s license and be insurable through FCPH’s provider; some evening and weekend hours required.
Hiring Salary: $20.00/hour to $24.06/hour. This is a non-exempt position.
